Q&A on Avandia Ban with Dr. Anoop Misra

Union health minsitry has banned the controversial anti-diabetic drug Rosiglitazone(marketed by GlaxoSmithKline as Avandia,and variant brand names produced by different pharma companies like Reddys, Sun, Glenmark etc.), based
on an expert committe recommendation. This recommendation is driven by the reasoning that the continued use of drug ups the chances of heart diseases and is particularly dangerous to Indians, due to their perceivced higher genetic pre-disposition to heart ailments. Given the prevelance of diabtes in India, this is a serious issue of public health concern.

I had sent out the question to a couple of experts on this field and gotten the response.The response from Dr. Anoop Misra here. Dr. Anoop is the head of diabetology at Fortis and was in the limelight on this issue.

1. Could you just take us through an overview of this recent news announcement of Avandia drug ban, focussing on what this drug is meant for, how widespread is its usage in India, what are the implications of this ban?

When introduced in 1999, this drug was believed to be excellent drug for diabetes since it was acting not only on blood sugar but also on liver and fat showing multiple benefits. Since then it has been regularly used as frontline therapy in diabetes. Till 2008, it had nearly 25% of share of all diabetes drugs in India, but since then the used has been declining due to reports of adverse reactions

2. Could you just give us a perspective on the evolution of drug development for diabetes and position the role of Rosiglitazone in this? Also, are there guidelines/framework to the industry on the development of drugs for diabetes and if yes, did the Rosiglitazone conform to all those guidelines?

By 2002, Thiazolidinediones (Troglitazone, Rosiglitazone and Pioglitazone) were at the center point in the treatment of diabetes. These drugs were developed according to strict drug development guidelines of FDA. However, at this time, FDA was not evaluating toxicity, if any, of any drug given on chronic basis, on heart.

3. Could you just give us a overview of the process that a new drug discovery needs to get through before it is approved to be marketed and is this process guaranteed to protect the interest of the patients? Does this process itself has inherent limitations and if so, how does it adress those?(It would help if you provide this explanation taking the example of Avandia itself)

Drug development process is long and tedious but not without shortcomings. After animal studies, phase 1, 2 and 3 studies are performed in limited number of human subjects to evaluate safety and efficacy. Most of these studies are of 3 months to at the most 2 year duration. These studies do not ensure that investigational drug does not cause side effect in larger number of patients after it is clinically used, since a particular side effects was not noticed in limited number of subjects. Further none of the studies could tell us what would be long term side effects of the drugs e.g. after its use for 5-10 years. The heart side effects of Avandia were not properly noticed till large number of studies were collectively analysed five years after it was first introduced.

4. Specific to this Avandia, it has been in the past removed from the market(US, through FDA ban) because of its unacceptable high risk of liver failure and later study found it to be safe? So does this mean that there could be a repeat phenomenon like this with the current ban too?

Any ban of any drug cannot be lifted. Please note that it was sister drug Troglitazone which was banned because of liver side effects and not Avandia.

5. Does the public confidence on regulatory bodies in protecting the patients interest and on the treating physicians on their choice of drugs get affected due to these kind of stories and what could be done to adress that?

Any drug regulatory body is not without its faults and bias. US FDA has clearly dragged its feet on Avandia, and strict action should have been taken long back. DCGI office (India) has much improved after current DCGI has taken over, however needs to be given much more support from Government with more powers.

6. Are there precedences in India, for such a ban on any chronic medicines? How did the post ban scenario span out in those cases, and could there be any learnings from that for this ban? Given the perceived high genetic pre-disposition to heart ailments do you think India has acted at the right time or they could have been more pro-active(Given the fact that Europe has done this ban couple of months ago and US has given restricted use recommendation a while ago)?

Please note that regulatory action from US FDA (strictly restricted use) and by European agency (Ban) has come only on 23rd September, 2010 not before that. Only black box warning was put on Avandia before this by USFDA. Considering this, India has clearly acted in proactive manner. Even before this, India was first country to ban ongoing trial of Avandia. Previously, Indian regulatory authority (DCGI) had reacted swiftly in similar manner for banning weight loss pill Rimonabant.

7. Now coming to the implications of this ban, What do patients who are currently on this drug do?, Suppose for a doctor who has prescribed this medicine to a patient for a long period of time and has clinical evidence to demonstrate the suitability towards this against other approaches (Intolerance to metformin kind of scenarios)(I know a "popular" diabetologist has explicitly stated that he has adviced a patient who is on this drug to continue and suggest that nothing will happen to his heart, though this interview was before the ban announcement) do in such a case? Are there suitable alternatives to this that poses less risk as against Rosiglitazone?

I cannot comment on other people’s practices, but our team had stopped using Avandia 2 years back, and I had emphatically lectured to various general practitioners and discussed with all prominent diabetologists in India to do so. We were never in doubt that this drug is not good for heart. Presently, there are a number of alternative drugs are available and any physician should be able to smoothly change from this drug to another without any loss of control. Even its sister drug, Pioglitazone could be used on its behalf since based on current data, it is safe.

The way forward for local television would be to engage citizens in debates on prominent problems of their city and make them part of the solution, Rajiv N. Lulla, CEO, NDTV MetroNation said on Wednesday.

In his keynote address on “Localism: The future of mass communication and what it means to you” hosted by the Chennai chapter of the Public Relations Society of India, Mr. Lulla said the key was in increasing citizen’s involvement in evolving solutions to common problems in everyday life.

Local television had the unique advantage of providing local content that was relevant to residents of a particular city that a national television channel with its limited “shelf-space” could accommodate, Mr. Lulla said.

For instance, a programme that invited viewer contributed videos would be impossible to undertake on a national scale but could be a great way to encourage citizen’s participation on local television, he said.

Accountable metrics

At the same time, local television needed to evolve an accountable metrics for advertisers who sought an alternative way to reach out to regional consumer segments, he added.

Preetha Reddy, Managing Director, Apollo Hospitals, said public relations was in essence all about the skill of communication and played a vital role in every organisation. However, the term was not always used in a complimentary manner, she said.

The Chennai chapter of PRSI also kickstarted its activities for the 2009-2010.

Naresh Trehan, brain behind ‘Medicity’: Interview and few thoughts on it

Naresh Trehan, an eminent cardiologist and the brain behind the Medanta Medicity project has answered questioned from a variety of people in the Indian Express this sunday. Following is the short summary on a few items that he has talked about along with my personal views on them.

On the motivation behind creation of medicity:

Creation of a healthcare facility that is patient driven, than target driven. A health care set-up which maintains a high level of transparency, in its governing practices, billing systems and patient outcomes. Also, on top a health care facility that is world standards but still accesible to the possible widest strata of the society and not just a select few.

His mention about publicizing patient outcomes and maintaining atmost transparency on this is a very important thing. Infact, he also mentioned that the outcome studies needed to be presented with a local context. Let me share a personal experience of mine, to better understand this point.

Once an aunt of mine had fractured her leg and there was a blood cot in the knee. We initially took her to a hospital to get the diagnosis, but for a variety of reasons we did not want her to be treated there and decided to move her to a bigger hospital in another city. Trying to hold us in the same hospital, the doctor told us about the risk involved with clots, that they have a tendency to move and can lead to brain stroke or cardiac arrest. We were scared of moving her from that places due to the risk. I then called up our family doctor and enquired about the risk involved in moving her. He said, there is definitely a small% risk involved, but I have not experinced in my medical carrer a patient where the clot due to hurting the leg has moved and led to a cardiac arrest, but you cannot come and blame me tomorrow if such a thing happens in your case because there definitely is a chance, but a chance event that I have not come across in my 15years of medical practice. So putting a statistical data into a context was re-assuring for us to evaluate the risk vs benefit that we gain by taking that risk.

Also on the target driven health care, unfortunately due to the heavy corporatization of the health care facilities in our country, doctors are functioning based on set target of having to accomplish a certain number of surgeries and whole lot per month to show corporate results. I hope this motivation as outlined in this inititaive will set a role model for other hospitals.

On the issue of affordable health care:

On a related note, he states that many of the medicinal practices that are followed in the country(on diagnosis, treatment approaches) are blindfoldedly followed based on the data available in the west. This is also an area where he states that MediCity would standout of the rest. I do not know whether use of indigineorus technology, and integrating other indian medicine practices also fir into his goal on affordable health care.

He says lot of health care cost burden can be brought down through dealing with it in the prevention phase. And the use of mobile health disbursal system to adress the rural health care needs.

I think his suggestion that satellite mobile medical mission with the help of qualified doctors instead of rural posting makes a lot of sense.

As regards the accountablity of the doctors his opinion was that doctors should be included in the professional regulatory body.

‘Possession’ is still very common, especially in rural areas. Even in Tamil Nadu, there are some temples such as Hanumanthapuram where I have observed a group of young women who used to remain in a so-called trance state for about 30 minutes around noon almost everyday. Even if considered a cry for help or attention, this practice gets social sanction and not perceived as a deviant behaviour.

Influenza A (H1N1) Revised Guidelines as on 14.8.2009

In order to prevent and contain outbreak of Influenza-A H1N1 virus for screening, testing and isolation following guidelines are to be followed:

At first all individuals seeking consultations for flu like symptoms should be screened at healthcare facilities both Government and private or examined by a doctor and these will be categorized as under:

Category- A

• Patients with mild fever plus cough / sore throat with or without body ache, headache, diarrhoea and vomiting will be categorised as Category-A. They do not require Oseltamivir and should be treated for the symptoms mentioned above. The patients should be monitored for their progress and reassessed at 24 to 48 hours by the doctor.

• No testing of the patient for H1N1 is required.

• Patients should confine themselves at home and avoid mixing up with public and high risk members in the family.

Category-B

(i) In addition to all the signs and symptoms mentioned under Category-A, if the patient has high grade fever and severe sore throat, may require home isolation and Oseltamivir;
(ii) In addition to all the signs and symptoms mentioned under Category-A, individuals having one or more of the following high risk conditions shall be treated with Oseltamivir:

• Children less than 5 years old;
• Pregnant women;
• Persons aged 65 years or older;
• Patients with lung diseases, heart disease, liver disease, kidney disease, blood disorders, diabetes, neurological disorders, cancer and HIV/AIDS;
• Patients on long term cortisone therapy.

• No tests for H1N1 is required for Category-B (i) and (ii).

• All patients of Category-B (i) and (ii) should confine themselves at home and avoid mixing with public and high risk members in the family.

Category-C

In addition to the above signs and symptoms of Category-A and B, if the patient has one or more of the following:

• Breathlessness, chest pain, drowsiness, fall in blood pressure, sputum mixed with blood, bluish discolouration of nails;

• Irritability among small children, refusal to accept feed;

• Worsening of underlying chronic conditions.

All these patients mentioned above in Category-C require testing, immediate hospitalization and treatment.

Guidelines for diagnostic laboratories: Swine Flu Testing

Guidelines for diagnostic laboratories pertaining to requirements of infrastructure for testing of specimens for Pandemic Influenza A (H1N1) virus infection (based on CDC/WHO guidelines1)

General Bio-safety measures: Gloves (latex), shoe cover, head cover, goggles, triple layered mask, mask with N-95 specification, front closed full length apron, puncture resistant autoclavable yellow coloured bio-safety bag with bio-safety symbols, hypochlorite solution. (As per bio-safety manual attached as Document 2).

Civil Infrastructure: Separate dedicated areas for sample handling and PCR testing as per recommended guidelines (Document 3).

Sample collection kit: Throat/Nasal swab with synthetic up (polyester or Dacron) and aluminium or plastic shaft sample collection vials or tubes (leak proof and autoclavable) containing 1-3 ml. virus transport media (with protein stabilizer and antibiotics) as primary container.

Sample storage: Refrigerator (4-80 C) for storage up to 48 hrs. Deep freeze (-700C) for longer storage.

Back up sample for future testing etc. should be kept at -700 C.

Sample Transport: Absorbent cotton, tissue paper or waste newspaper for wrapping primary container. Secondary container to hold the primary container i.e. bigger tube or sealed plastic bag. Insulated ice box with ice pack, sample proforma fastened on to the secondary container.

Sample handling and testing:

Handling: In BSL-3 Bio-safety or BSL-2+ facility with BSL-3 precautions.

Testing: Real time RT-PCR test, by Real Time PCR machine using validated reagents accessories and protocol as per CDC/WHO guidelines/testing protocols3 and subsequent amendments published from time to time.

Reporting of Results: Standardized uniform reporting proforma. PC with internet facilities, fax machine.

Lack of clear-cut swine flu guidelines, to citizens and private doctors, by Municipal Corporation and health officials resulted in unnecessarily long queues in Naidu Hospital. Moreover a lot of confusion continues amongst lay people and doctors about specific steps to be taken.

There are two criteria for throat swab collection and its testing at National Institute of Virology (NIV) and those are: only those patients with cough, cold, fever who had been a) to foreign countries in last eight days or b) in contact with swine flu patients. However this was not made sufficiently clear through newspaper, radio and TV by Municipal Authorities leading to overcrowding at Naidu Hospital.

Even persons not fitting in above two criteria are also getting swine flu infection. Hence there is need to change these criteria and new ones should be properly publicized through media. Even today, there is no clear guideline for private practitioners and doctors. On the contrary on 6th August, government issued a threat of registration cancellation to doctors, through newspaper if they did not take proper care of patients, without making it clear what does it mean to take proper care!

Municipal Corporation printed more than one lakh information pamphlets. But it contains some incomplete and faulty instructions. E.g. ‘wash hands continuously’, ‘avoid congested places’. What does it mean to citizens? Some instructions have been taken from ‘Centre for Disease Control’,USA website. However CDC website says keep 6 feet distance from swine flu patient, PMC pamphlet says only 3 feet! Despite conveying these corrections to concerned health officials, there is no response from them.

No clear publicized information is provided to public and doctors on why only Naidu Hospital is collecting samples and only NIV is testing them, why only Naidu Hospital is providing required medicines.

Directorate of Maharashtra Health services has issued impracticable and vague instructions to doctors in today’s Indian Express.

Jan Arogya Abhiyan demands an immediate end to this indifference by Municipal and Health authorities. There is need to issue proper, scientific instructions for public and doctors through all newspapers and display them frequently on all television, radio channels.

Who one should visit doctors and when? When to directly approach Naidu Hospital and other 15 screening centres? What kind of congested places should be avoided? Who is at greater risk of infection? Who should use mask? What is exact meaning of ‘contact with swine flu patient’? Does it mean spending whole day with such patient or just brief time? Is it necessary to sterilize entire class or office if there is any swine flu patient? People should get proper scientific and specific answers to all such questions.

Private medical practitioners and civil society experts should be consulted while preparing guidelines for public and doctors.

Swine Flu update: Revised Guidelines from Health Minstry

Revised Guidelines for testing of persons with flu like symptoms reporting at hospitals notified for influenza H1N1

So far, the present guidelines stipulate that a person suspected of influenza A H1N1 need to be referred to an identified govt. health facility. He/she needs to be kept in an isolation facility in that hospital and if found positive, is treated accordingly.
In order to make the testing facility for H1N1 more accessible at large and due to the onset of the Influenza season in the country, it has been decided to revise the existing guidelines.
Under the new guidelines, any person with flu like symptoms such as fever, cough, sore throat, cold, running nose etc. should go to a designated Government facility for giving his/her sample for testing for the H1N1 virus. After clinical assessment, the designated medical officer would decide on the need for testing. Except for cases that are severe, the patient would be allowed to go home (This was not allowed under the existing guidelines).
The sample of the suspect case would be collected and sent to the notified laboratory for testing. If tested as positive for H1N1 and in case the symptoms are mild, the patient would be informed and given the option of admission into the hospital or isolation and treatment at his own home.
In case the patient opts for home isolation and treatment, he/she would be provided with detailed guidelines / safety measures to be strictly adhered to by the entire household of the patient. He/ she would have to provide full contact details of his entire household. The house hold and social contacts would be provided with the preventive treatment.
Notwithstanding the above guidelines, the decision of the doctor of the notified hospital about admitting the patient would be final.

In case the test is negative, the patient will accordingly be informed.
These guidelines have been issued by the Government in public interest and shall be reviewed from time to time depending on the spread of the pandemic and its severity in the country. These guidelines would however not apply to passengers who are identified through screening at the points of entry. The existing policy of isolating passengers with flu like symptoms would continue.

Swine Flu Scenario in Pune

The number of death victims associated to Swine Flu has touched 6(it was 5 when I started typing this) and the scare is quite palpable amongst the citizens of Pune. I have been carefully observing the developments in the last couple of days and wanted to register some of my thoughts on this issue.

First and foremost of the concerns is that it has been more than a week and the government is yet to come with the defined protocol to follow while dealing with Swine Flu. It is understandable in a situation like this, the government cannot work with some pre-set guidelines and needs to condition their strategy and evolve different protocols as the situation demands.

Just the day after the first victim had died, the government attributed it to not approaching the designated health care facilities in time and losing valuable time by consulting the private medical practitioners. In-fact, one of the health department official(I think it is the health minister of Maharashtra) went as far as telling that licensing of the private doctors will be cancelled if they treat someone showing swine flu symptoms.

Just a day or two after that, government is suggesting that private doctors needs to treat them and should not send them away. The ambiguity and dilemma in defining the strategy to deal with how situation evolve is understandable. But should not they deal with some forethought. I mean if within a couple of days, one imagines a scenario of engaging private medical professional in handling the crisis, what is the need to take such an aggressive posture.

And the information also seems going back and forth on all the issues associated, like administering Tamiflu to suspected individuals and everything. The teacher who lost his life in pune was asked to go back and told that he is not a possible suspect by the same government doctors when he has approached them. Such cases are in the rise, so the diagnosing doctors decision whether to send the sample for testing or not is still very ambiguous.

Even for simple things like procuring the masks to protect against the virus, the special protection masks, which are not so easily available at-least needs to be made available to the high risk people such as the pregnant women or other ailing people who are more risk prone and are under medical care and more susceptible to medical infection.

Also with the number of death counts increasing, it is a issue of serious concern, whether the government has any strategy on how intensive and critical care departments will cope with high patient numbers.

It is not just enough to request people not to panic, without making adequate actions that would make them feel confident of the situation.

As regards to spreading the awareness and establishing the proper information to communicate, the fundamental issue as stated in the earlier paras are to establish and define, clearly what needs to be communicated.

One of the doctors on TV, suggested that information spread through radio does not seem to be happening at all, I think all media tools like SMS, FM radio channels all need to be channelized for the information spread. Fortunately it is reported that the spread of the Flu is not large in rural areas, in the event of such a spread, the information spread through Radio needs to be effectively used.

I will keep posting on whatever that in the course of time thoughts I have on this issue that needs to be shared with.

We have an advantage that we have abundant natural resources in our country,this including a talent pool of educated people, literate people and uneducated people with wordly wisdom.

We also have several advantages like booming sectors in flourishing economy temporary minor hiccups not taken into consideration ,a maturing entratainment industry which we relish in our spare time, a expensive but renowned health care industry, network of educational establishments churning out street smart kids.
We also have a third or second largest economy in the making within a span of 15-20 years.

During the election campaigns in US, we closely followed Mr.O'bama's campaign and was thrilled by his caption "yes we can". Have we wondered whether we too can have this feeling of "yes we can"?

My following text is to highlight a pressing need of the hour for a silent but non-revolutionary awakening among us for a minor change/amendment in our political system. The makers of our constitution who have been geniuses in coming out with a fine blue print for democracy have but failed on 2 counts. I make this statement with due respect to our makers of our constitution , but with a humble spirt i make these observations.

During the 26/11 attacks we watched our youth spearheading a campaign mouthing slogans of delhi chalo to wake up our leaders in the parliament to pressing need for effective security techniques in the country. i could see in the tv , a spirt of bapu, a dynamism of nehru, a steely resolve of sardar, a determination of bhagat amongst the mindset of the youth.

Coming to brasstacks, we see a profileration of parties based on ideologies,caste based ideologies. While the democracies in US and UK with thier two party system are able to move ahead peacefully without any confusion of sorts, we are not able to do so in a streamlined manner. The reason being that during elections the voter is not able to make a clear choice in his descion , since there is a plethora of parties for a choice to be made and well as leaders of parties are confused and seem to make alliances based on permutations of future coalition parties.
Is it possible for the younger generation among us, we the people in our generation to make a campaign assertively stating that "yes we can" for a 2 party system. his would solve most of the confusion in our country as a first step.Could we press our leaders for a 2 party system at the center by making minor amendments to that effect. In this way caste based politics wont be there , in first step and the voter would not only choose between the two of parties at the center. Once this goes successfully , the success plan at the center would percolate at the states, where finally a 2 party system is also at the state.
I request our friends to think deeply with existing clarity and make up thier minds so that future generations dont go through the confusions of past and present generations.
A very simple solution being that the major and second major party could be taken as 2 parties , and remaning parties made to join these parties , either the largest or second largest party. By largest i mean in terms of party membership.
AN e.g from my humble view is that we have parties , largest being congress and the BJP, the other parties willing to form a coalition stake could fuse themselves into congress or BJP, thus dissolving thier party indentities.
I think from my humble opinion that this is way for clear cut electoral policies as well electoral choice as is prevalent in US or UK.

It was morning and the day was getting hotter,by hotter I mean peoples emotions
were getting fiery. There were news of clashes between 2 castes,there was a news spreading by way of hearsay that a strike had gone violent.In another part of the country there were rumors of regionalism raising its ugly head and a couple of people had been roughed up in that region on grounds that they were not born in that native place!!.

In addition the suffering old men and women were trudging on barefoot, this was the army of hungry old people trudging on bare foot expecting a meal or two from the flashily dressed people driving fashionable cars and bikes.. thier pumpled egos and fantasies driven up by movies and the feel good factor that economy was booming for them. An old lady begging for food was completely ignored by a set of young men and women poised in couple of tables on a road side famous eatery. The old lady clutched her under belly in deep hunger and the muscles in her abdomen were corroded by the gastric juices..symptoms like fainting and low blood pressure were assailing her senses and she had to endure this suffering of going without food for this day too..

Few couple of lovers were seen handing thier hands and enjoying the fleeting moment and time,oblivious of the scene that a young infant had been let to handle crumbs of breads in roadside while the mother was seen begging in the fleet of cars waiting at the signal which was about to turn yellow(those were the good old days..Mumbai Express!!!).

In a remote region of the country 2 sets of regious people were seen fighting with one and another. Blood was running freely on the roads and pavements. Children were burnt alive. Young men were beheaded while young ladies were raped. The rapacious nature of both the sets of relegion forgetting that there could be possibility of GOD being one if there was one GOD, and it was just the matter of religions being different.

All the events were paradoxical to fact that there had lived a man who had belonged to humanity.This man was frail in anatomy but had led crores people in a non violent struggle from his place called Sabhartamti. He had had been called Bapu and was one of the first ever social prophets.His sayings were much respected and admired by the western world. People had forgotten this social prophet called Bapu in his own land (and dancing to the tunes of Bapu.. Bapu..).

Preliminary draft of my work on Patient Rights

Ashwin1 was diagnosed of having acute appendicitis and advised to undergo appendectomy (surgical procedure to remove appendicitis) immediately. But for the slight discomfort the previous week, he felt his health was quite normal and can't understand the need to undergo a surgical procedure at such a short notice. Not wanting to take chances, felt that he will probably have a second opinion from another physician on his condition and the need for an immediate surgery. He requested the hospital authorities to share with him the medical reports so that he will have a second opinion on the suggested course of treatment. He was surprised to find the indifferent attitude of the hospital towards him and their unwillingness to part with information to seek another opinion..

Aditya's 2 3year old son was prescribed a drug overdose by a pediatrician for the recurring fever. This led to very severe consequences and the child needed ICU care for 3 days and a painful process of recovery due to this wrong medication. Aditya wanted to register his complaint against the pediatrician and the hospital but finds out that there is absolutely no framework for grievance redressal and there is a systemic neglect to have one. The prevalent attitude with the people managing the health care set-up's is that such human errors are an inevitable artifact of the Indian landscape and there is not a need to address them and the hospitals do not have a legal or a moral responsibility to address them..

65 yr old widowed Chitra3 was admitted for a by-pass surgery in a leading multi-specialty hospital. She gathered information about the treatment costs and was given an estimate of the cost involved with the treatment and decided to go ahead with it. But the hospital stated in the middle that due to unexpected complications that came up during the surgery charges have become significantly higher than what she was told in the beginning. Neither she nor the ones who were caring for her were even briefed of such a possibility at any point in time before the start of the treatment. The difference in the rates were so significant that Chitra may not be able to meet that expenditure, which puts her in an un-enviable position of having to choose whether to continue the treatment or not in that hospital.. for not being able to afford it..

Does any of this sound too familiar an experience to relate to what you or your loved ones face while dealing with the health care set-up? Then you have certainly hit upon a web-page, which makes an earnest attempt at addressing one of the fundamental aspects of the complex issue, which has led the health care system to what it is now.

Traditionally Doctor-Patient relationship in our culture has worked on the tenets of basic faith and trust on doctors. In earlier times social accountability (the possibility of earning a bad name due to improper medial advice) and the very dynamics of society which offered scope for a personal relationship with doctors treating or managing the hospitals in towns and cities provided a framework where-in the ability of doctors to self-govern themselves was thought of to be sufficient. However in these times of crass commercialization and corporatization of health care system in our country, the above line of thinking has lost its relevance.

To ensure that the patient's interests are best served in this system, we need a well defined regulatory mechanism that is defined to the minutest details on its implementation and larger consumer participation towards ensuring its implementation. Fundamental to the realization of any of our aspirations on that front is the articulation of the fundamental rights that a patient is entitled to in this system.

A pertinent point to note here is that 87%(search in net or collect this info from Dr. Abhijit and put the appropriate number here) of the health care service in India is offered by the private players. The continuing systematic decline of the state run health care institutions is going to further increase this number in the coming years. These private health care players can range from anything like a single person managed OPD clinics, individuals owned 20-30 bed nursing homes to corporate run multi-specialty hospitals. With the broad spectrum of interests that drives the private players in the health care system, without a legal obligation for compliance to set rules and punitive measures for its dereliction, it is almost impossible to bring in any rational framework to make this wide variety of players comply with the basic rights that the patients are entitled to. The focus of this web-page is to mobilize public support towards a movement aimed at achieving this.

While various organization and bodies in the past have focused on the issue of patient rights in the health care system, the efforts have been quite scattered and invariably targeted at certain specific interest groups (E.g. National Inst of Mental Health and Neurological Sciences (NIMHANS) focusing on the rights of mental health patients, National Aids Control Organization (NACO) guidelines for the rights of the HIV infected). SATHI CEHAT working under the auspices of Guan Maharashtra(Re-name to the appropriate body) has worked to evolve a standard charter for the patient rights. This provides a comprehensive set of the basic rights and also defines the mechanism for the functioning of health care set-up that would facilitate the establishment of these rights for the patients. The developed charter now needs the states approval to formulate them as mandatory rules to follow for the hospitals. The charter is focused on the following:

Right to access health care
Independent of their caste/creed or economic status has the right to health care
Right to non-discrimination
In the case of HIV-infected or other illness where there is a social stigma attached to it.
Right to emergency treatment
Right to Information
Share information on all aspects of treatment
Nature of illness
Complications that can arise out of treatment, cost and expected outcomes
Alternative treatment options available
Consequences of not choosing to take the treatment
Access to his/her medical records at all times
Discharge/Death summary at the time of leaving the hospital
Right to privacy and confidentiality
Right to autonomy and decision making
Right to seek a second opinion
Right to have a grievance redressal forum
Right to have a mechanism to make informed consent
This is applicable to nursing homes involved in clinical trials

Though the issues and rights based discourse that are outlined above has a pan India relevance, the ground level activities of CEHAT are targeted at getting this charter of rights established at the hospitals in Maharashtra. This requires that this charter gets the approval of the Maharashtra state government (as health is usually a state subject).

சர்க்கரை நோய், என்று சொல்லப்படும் Diabetes இன்று மனிதர்கள் எதிர் கொள்ளும் உடல் பிரச்சனைகளில் முக்கியமான ஒன்று. அதிலும் India is Diabetes's world capital என்று சொல்லப்படுகின்ற அளவில் நம் நாட்டில் இந்த நோயின் பாதிப்புக்கு ஆளானவர்களின் எண்ணிக்கை அதிகமாக உள்ளதாக புள்ளி விவரங்கள் தெரிவிக்கின்றன. இந்த சர்க்கரை நோயை சமாளிப்பது எப்படி என்ற சிந்தனைக்கு பதில் கூறும் நோக்கத்தில் "சர்க்கரை நோய் சமாளிப்பது எப்படி" என்ற தலைப்பில் Dr. Maruthupandian(எழுத்து வடிவம் அய். ஜெயச்சந்திரன்) எழுதிய(!) இந்த புத்தகத்தை கிழக்கு பதிப்பகம் வெளியிட்டுள்ளது(விலை ரூ. 80). இந்த புத்தகத்தை படித்த பின்பு இந்த புத்தகம் தொடர்பாக எனக்கு தோன்றிய எண்ணங்களையும் சிந்தனைகளையும் பகிர்ந்து கொள்ள எண்ணும் ஒரு முயற்ச்சியே இந்த பதிவு ஆகும்.

முந்தைய paraவில் "எழுதிய" என்று ஆச்சரிய குறியிட்டதன் காரணம் என்னவென்றால், எனக்கு இந்த எழுத்து வடிவம் என்ற சொல்லின் அர்த்தம் சரியாக புரியாததே ஆகும். இந்த புத்தகத்திலும் அதன் விளக்கம் எதையும் நான் பார்க்க முடியவில்லை. என்னுடைய understanding என்னவென்றால் there is an abundant wealth of technical(includes medical)/field specific knowledge that needs to be translated(I dont just mean the language here) and be made accesible to a larger population and are these "எழுத்து வடிவம்" some sort of facilitators for such an exercise. In the sense these are set of people, who try to digest the technical details involved through dialogue with the field experts and translate the information into a written/literary form. இந்த புத்தகத்தில் கூறப்படும் "எழுத்து வடிவம்" இந்த வகையை சார்ந்த்தா?, தெரியவில்லை.

முதலில் இந்த புத்தகத்தின் மையக்கருத்தாக Dr. Maruthupandiyan சொல்லும் விஷயம் என்னவென்று பார்போம். இந்த சர்க்கரை நோயானது Chronic Illness வகையை சார்ந்தது, அவை Acute Illness வகையை போல தோன்றிய உடன் கிடு கிடுவென வளர்ந்து உடலுக்கு தீங்கு விளைவிக்காமல், நம் உடலிலேயே நீண்ட காலம் உரைந்து பல வித நோய்களுக்கும் ஊற்றுக்கண்ணாக இருக்க கூடிய ஒரு நோய். ஆனால் பெரும்பாலனோருக்கு நினைத்தால், உரிய வாழ்வியில் மாற்றங்களை ஏற்படுத்தி கொண்டால் இந்த நோய் வராமல் தடுக்கவும், அப்படியே வந்து விட்டாலும் அதன் தாக்கத்தை வெகுவாக குறைக்கவும் சாத்தியம் உள்ள ஒரு நோயாகும். அப்படி ஒரு நிலை ஏற்படுவதற்க்கு இந்த நோய் பற்றிய முழுமையான அறிவும், விழிப்புணர்வும், கட்டுக்குள் வைக்க தேவையான தகவல்களும் அறிந்து வைத்தலும் உதவும் என்ற நம்பிக்கையும், அந்த நம்பிக்கையை செயலாக்கும் நோக்கத்தில் உருவானது தான் இந்த புத்தகம் என்றும் சொல்கிறார்.

சர்க்கரை நோய் பற்றிய ஒரு எளிய அறிமுகத்துடன் தொடங்கி, அது யார் யாருக்கு வரக்கூடிய சாத்தியங்கள் அதிகம் என்று விளக்கி, அந்த நோயின் தன்மை, மற்றும் பல வகைகளை என்ன என்று எடுத்து கூறி, நோய் தோன்றலின் அறிகுறிகள் எவை என்றும், அந்த அறிகுறிகளை தொடர்ந்து அதை உறுதி படுத்தி கொள்ள தேவையான பரிசோதனைகள் என்ன என்ன என்று விளக்குகிறது புத்தகத்தின் முதல் பகுதி. இதன் இரண்டாம் பகுதியில் இந்த நோயின் பொதுவான பாதிப்புகள், மற்றும் இந்த நோயினால் ஏற்படக்கூடிய நீண்ட நாள்பட்ட பாதிப்புகள் ஒவ்வொன்றையும் எடுத்து கொண்டு விரிவாக விளக்க படுகிறது. இந்த நோய் பராமரிப்பு பட்ற்றியும், மேற் கொள்ள வேண்டிய உணவு முறை, உடற் பயிற்சிகள் பற்றியும் விளக்க படுகிறது. இதனை தொடர்ந்து சில நோயாளிகளின் கேள்விகலுக்கு டாக்டரின் பதில்களும் இந்த பகுதியில் இடம் பெருகின்றது. அதன் பின் பிற்சேர்க்கையாக சில உபயோகமான(உணவு அட்டவணை, பரிசோதனை முறைகள் ஆகியவை) தகவல்கள் வழங்க பட்டுள்ளன.

முதலில் இந்த புத்தகம் ஒரு டாக்டராலேயே எழுதப்பட்டிருபது இந்த புத்தகத்தில் உள்ள தகவல்களுக்கும் கருத்துக்களுகும் ஒரு நல்ல நம்பகத்தன்மையை ஏற்படுதுகின்றது. நோய் பற்றிய அறிமுக பகுதிகளும்(குறிப்பாக கனணையம் செயல்பாடு குறித்த முதல் பகுதி விளக்கங்கள்), ஒரு மேம்போக்கான விளக்கமாக இல்லாமல், முழுமையான நோய் குறித்த அறிதலை ஏற்படுத்த வழி வகுக்கும் வகையில் அமைந்து உள்ளது இந்த புத்தகதின் சிறப்பு. அதே நேரத்தில் புத்தகத்தின் பல இடங்களில், குறிப்பாக நாள்பட்ட விளைவுகளை ஒவ்வொன்றாக எடுத்து விளக்கும் பகுதிகளில், டாக்டர் தன் நோயாளிகளின் நடந்த உண்மை சம்பவங்கள், special situations ஆகியவற்றை பகிர்ந்து கொண்டிருந்தால் இன்னும் பயனுள்ளதாக அமைந்து இருக்கும். இந்த குறை ஒரளுவுக்கு கேள்வி பதில் பகுதியில் நிறைவடைகிறது.

Diabetes வராமல் தடுத்து கொள்ளவும், வந்த பின் காத்து கொள்ளவும் ஒரு பன்முக அணுகுமுறை தெவை படுகிறது. அவைகளில் உணவு முறை, உடற் பயிற்சி இரண்டும் முக்கியமான முகங்கள். இந்த உணவு முறைகள் என்பது ஒரு region specific item. அதனால் ஆராய்ச்சி கண்டுபிடிப்புகளை நம் உணவு முறைகளுக்கு apply செய்து பார்ப்பது அவசியமாகிறது. For example Glycemic Index of the food is considered an important aspect in deciding on the food habits of the diabetic. This glycemic index is typically associated to a specific food product(Rice has a specific GI, whilst Wheat, Pulses has a different GI). Unlike, most of the western diet, Indian food grains or pulses(Protein intensive) are not eaten in isolation. Even our Idli or Dosas are a combination of Rice and Pulses, unlike the western staple diets like wheat potatoes, hence interpertation of diet based on GI is a little tricky in our context). இந்த மாதிரி இந்த நோய் அணுகுமுறைகளுக்கு ஒரு வித region/context specific approach பற்றி கூற பல விஷயங்கள் உள்ளன. இவை இந்த புத்தகத்தில் address செய்ய பட்டிருபதாக நான் உணரவில்லை. அதே போல இந்த உணவு முறை பகுதிகளை ஒரு dietician contributory articleஆக எழுதி இருந்தால் இன்னும் பயனுள்ளதாக இருந்திருக்கும் என்று நம்புகிறேன். இந்த கருத்து உடற் பயிற்சி குறித்த பகுதிக்கும் பொருந்தும்.

புத்தகத்தில் முக்கியமாக நாள்பட்ட விளைவுகள் குறித்து விளக்கும் இடங்களில், மேலும் சில விளக்க படங்கள் கொடுதிருந்தால் உபயோகமாக இருக்கும். அதே போல பிற்ச்சேர்க்கை பகுதிகளை கூடுதல் பக்கங்களாக சேர்க்காமல், diet, Calorie charts ஆகியவற்றை சமையல் அறையில் ஒட்டி கொள்ள கூடிய அட்டவணை வடிவில்(ஒரு உதாரணதிர்க்கு) சேர்த்தால் பயணுள்ளதக இருக்கும். அதே மாதிரி handling low sugar emergencies பற்றிய குறிப்புகளை ஒரு கையடக்க புத்தக இணைப்பாக வழங்கினால் பயனுள்ளதாக இருக்கும் என்று நம்புகிறேன்.

To summarize, inspite of the fact that I believe that there are things whih ould have been dealt differently, this book does provide a very comprehensive knowldege on issues related to diabetes and as to how to deal with it as an illness.

Visit to Ralegaon Siddhi

I made a one day visit to Ralegaon Siddhi on the 1st October 2008. This is a model village(Adharsha Gaon), very popular for the water shed management and the self sufficiency models that were implemented for the village welfare. Anna Hazare lives here in a small temple.

I will just provide answer to a few question which I had when I decided to make this trip and did not find enough resources on the web to answer them.

The questions are

How do I reach this place by bus from Pune?, How long does it take?.

There are few direct bus from Shivaji Terminus in Pune to Ralegaon Siddhi. This service is very infrequent and seems to be available only for a few times in a day. Alternatively the option is to take a bus to Sirur. This small town is about 60kms from Pune and most buses to Ahmed Nagar passes through Shirur. The state road transport buses are decent, but not very comfortable. The ticket costs 47Rs. It takes about one and half to two hours to reach Sirur from ST bus terminus Pune. Enroute is this famous Ashta Vinayak temple town called Ranjan Gaon and it is easy to make a stop over if one is interested. The frequency of services is plenty, almost every 5 minutes you have a bus going to Shirur.

After you reach Sirur, one needs to take a bus to Parner and Ralegaon Siddhi(people understand if you say Anna Hazare gaon also)is on the way to Parner. This travel takes about 20-30 minutes. There are 6 seater tempo services also on this route, but the frequency might be a issue. Buses are available once every half an hour. The ticket cost 10Rs.

How do I go around looking for things in the village and will there be some guided trips explaining things?

National water shed management training institutes organizes guided trips. This center is about half a kilometer from the place where you get down from the bus. This trip is typically offered for groups and people who have their own vehicle. If you dont have a vehicle, which was the case in my situation they do not provide any transport to visit those water sheds which are located at distances of about 2-3kms within the village.

If you are a group and call them on phone(02488-240401) and have your own vehicle, this might be the best option to get to understand their watershed managment initiatives.They also provide lunch for the visitors at a reasonable price. My personal experinece was that they were not so enthusiastic or forthcoming to showcase and explore options to make this trip available for individual visitors interested.

In my case as this trip was not possible, I visited the media center, which is a photo exhibition explaining and showcasing the village activities towards developement and spent some time there. Made some conversations with the locals(not knowing Marathi and not very conversant with farming terms in Hindi was a handicap to understand quite a few things they were talking to me) at a chai hop and told them my interest to visit those water shed places and lack of vehicle from the training center.

The locals were forthcoming and very glad to offer to take me around in their motorbike(I offered that I could come in bicycle too) and one of them took me around the village. He explained me about their farming practices, watershed managment initatives(Technically it was hard to get the efficiency of this practice and how does one manage such a thing in a topologically different kind of village, how do they manage to avoid mosquito breeding in those waters that get stagnated in different places was difficult to understand and a few other assorted queries, but there must be plenty of resources in the web to understand them). It was also impressive to know that a villager typically makes about
25 to 30 thousands per annum per acre through farming. I know that in cauvery delta in tamilnadu most farmers hardly make about 10,000. I was glad that I made this trip with a local(He works for a village bank, which lends through a co-operative kind of network(not sure if it is a micro finance initiative, but the high interest rate of 15% that he mentioned made me think that it i probbaly that sort of a model) managed by Anna Hazare, who was really showing a lot of pride in showcasing the success of his village and demonstrated a keennes to explain to a outsider. The staff at the training institutes did not give me the impression that they would make this kind of a feeling to their vistors and were doing like more like a ritual.

Anna Hazare was not in the village when I visited. But I visited the temple where he stays. On the whole it was a valuable experience.

AML மேல் மேல் மேல் புற்றுநோய் சில சில அணுக்களே பற்றி சிறு குறிப்புக்களை. AML காணலாம் புற்று நோய் சிலவற்றை குறிப்புக்களைகாணலாம்lymphocyte குறிப்பகளை காணலாம்தன) மாற கூடிய ரத்த அணுக்களிலேயே தோன்றுகின்றன. Lymphocytes ல் தோன்றுவன ALL(Acute Lympocytic Leukemia) என்று வேறொரு புற்று நோய் வகையாக கருதப்படுகின்றது.

AML பொதுவாக BoneMarrow என்று சொல்லப்படும் எலும்புகளின் மிருதுவான உட்பகுதியான, உடம்பில் மண்டை ஓடு, தோல் பட்டை, Pelvis முதுகெலும்பு ஆகிய இடங்களில் உள்ள ஒன்றிலியே தோன்றுகிறது. அவை அங்கே தோன்றினாலும் வெகு விரைவிலேயே மூளை, விரை மற்றும் இரைப்பை ஆகிய இடங்களுக்கு பரவகூடிய்வை. பொதுவாக உடலில் வேறு உறுப்புகளில் தோன்றி BoneMarrow க்கு பரவிய புற்று நோய்களை AML வகையில் சேர்ப்பது கிடையாது.

சிகப்பு ரத்த அணுக்கள்:: இவை பிராண வாயு(Oxygenஐ) lungsலிருந்து உடலின் எல்லா திசுக்களுக்கும் எடுத்து சென்று, அமில வாயுவை(Carboசிகப்பு ரத்த அணுக்கள்:: இவை பிராண வாயு(Oxygenஐ) lungsலிருந்து உடலின் எல்லா திசுக்களுக்கும் எடுத்து சென்று, அமில வாயுவை(Carbon-di-oxideஐ) திரும்பவும் lungsக்கு எடுத்து செல்லும் முக்கிய பங்கு ஆற்றுகிறது. இதில் குறைபாடு, அதாவது இந்த வகை ரத்த அணுக்கள் உடலில் குறையும் பொழுது தேவையான பிராண வாயு திசுக்களுக்கு கிடைக்காததால் உடல் சோர்வு மட்ற்றும் மூச்சு வாங்குதல் ஆகிய பிரச்சனைகள் உடலில் ஏற்படுகின்றது. இதை Anaemia(ரத்த சோகை) என்று சொல்வார்கள்.

Platelets:: உடலில் வெட்டு அல்லது சிராய்ப்பு ஏற்படும் பொழுது ரத்த நாளங்கலில் ஒட்டை ஏற்பட்டு ரத்தம் வெளியேரும், அதை அந்த ஒட்டைகலை அடைக்கும் பணியை செய்வது இந்த plateletகள் தான். இவை அளவு கம்மியாகும் பொழுது, CBC(Complete Blood Count) testல் இதன் அளவு ஒன்றரை லட்சத்துக்கும் குறைவாக இருந்தால் அதை Thrombocytopenia என்று குறிப்பார்கள். ரத்த நாளங்கல் அடைபடாத காரணத்தால் கட்டுகடங்காமல் ரத்தம் உடலை விட்டு வெளியேரும் அபாயம் உண்டு.

வெள்ளை ரத்த அணுக்கள்:: உடலை தாக்கும் நோய் கிருமிகளிலுருந்து தற்காத்து கொள்ள இவையே முக்கிய பங்கு வகிக்கின்றன. மேலுள்ள விளக்க படத்தில் குறியிட்டது போல, இந்த வெள்ளை ரத்த அணுக்கள், ரத்த மரபணுவில் இருந்து பிரித்து குறிக்கப்படும் இரண்டு வகைகளான Myeloid stem Cells and Lymphoid Stem cells இரண்டு வகைகளிலும் இந்த வெள்ளை ரத்த அணுக்களை குறிக்கின்றன. இந்த இரண்டு வகைகளிலிருந்து உருவாகும் வெள்ளை அணுக்கள் முறையே Granulocytes and Lymphocytes என்று கூறப்படும். இதில் Granulocytes என்பவை microscope வழியாக பார்க்கும் பொழுது பொட்டு போன்ற உருவில் இருக்கும் அணுக்களை குறிக்கும். அந்த பொட்டின் அளவு மற்றும் நிறத்தை பொருத்து அவை மேலும் வகைப்படுத்தபடுகின்றது.

இந்த ரத்த மரபணுவிலிருந்து தோன்றும் மேல் விளக்க படத்தில் உள்ள வகை ரத்த அணுக்கள் தன் பரிணாம வளர்ச்சியில் சாதாரண இயல்பிலிருந்து மாறி Leukemia அணுக்களக மார வாய்ப்பு உள்ளது. அவை அப்படி தோன்றி அங்கேயே சாகாமல் கூடி பின் ரத்ததில் கலந்து மற்ற உடல் உறுப்பு அணுக்களில் கலந்து அவைகளையும் தன் இயல்பில் வேலை செய்ய விடாமல் செய்யும்.

ரத்த புற்று நோயின் பல வகைகளை புரிந்து கொள்ள உதவும் பொருட்டு, உடலில் ரத்த உற்பத்தி செய்யும் உருபுகளை பற்றியும், ரத்த அணுக்களின் வகைகளை பற்றியும் அறிந்து கொள்வோம்.

Acute Myeloid Leukemia என்பதை குறுக்கி குறியிடும் சொல்லே AML. இதில் Acute என்பது கவனிக்க பட வேண்டிய வார்த்தை. நோய்களை அதன் தீவிரமடையும் தன்மையும் அதற்காகும் கால அவகாசத்தையும் ஒட்டி Acute or Chronic illness என்று இரு வகைப் படுத்தப்படுகிறது.
Diabetes, Hyper Tension மற்றும் சில வகை புற்று நோய்கள் கூட chronic illness வகையை சார்ந்தவை என்று கூறுகின்றனர். இதன் அர்த்தம் இந்த வகை நோய்கள் உடலிலேயே சில பல வருடங்கள் தங்கி அதனால் ஏற்படுத்தக்கூடிய உடல் ரீதியான தாக்கங்களை ஏற்படுத்தவல்லது. ரத்த அழுத்தம் உள்ளவர்களுக்கு அதன் விளைவாக இருதயம் மற்றும் சிறுநீரகம் பாதிப்புக்கு உள்ளாகும் என்பது ரத்த அழுத்த நோய் தோன்றி சில/பல வருடங்களுக்கு பிறகு அவை கட்டுப்படுத்தபடாமல், கவனிக்கபடாமல் இருந்ததால் அவை உடலிலியே உறைந்து இந்த விளைவுகளை ஏற்படுத்த வல்லது.

இதற்கு மாறாக Acute வகை சார்ந்த பிரச்சனைகள் சில நாட்கள் மாதங்களிலேயே அதன் தீவிரம் அதிகரித்து உருபிகளின் செயளிழப்போ, உயிருக்கே கூட ஆபத்தோ கூடியவை. இந்த மேல் சொன்ன விளக்கங்களுக்கு இந்த ரத்த புற்று நோய் விஷயத்திலும் Acute Myeloid Leukemia and Chronic Myeloid Leukemia என்று இரு வகை உள்ளன.

இதில் Acute வகை தோன்றிய சில மாதங்கிளிலேயே கவனிக்க படா விட்டால் உயிரையே மாய்க்க வல்லது. ஆனால் சில நேரங்களில் chronic வகையோ சில நேரங்களில் நோய் தீவிரம் அடைய ஏழு எட்டு வருடங்கள் குட`ஆகலாம் என்பதால் அதை உடனே சிகிச்சை செய்து தொந்த்தரவு செய்ய வேண்டாம் என்று கூட முடிவெடுப்பது உண்டு.

ரொம்ப காலமாக செய்ய நினைத்து தள்ளி போட்டு கொண்டிருந்த விஷயம் இது, தள்ளி போடலின் முக்கிய காரணம் மிக சரியான அணுகுமுறை இதுவோ, இல்லை அதுவோ எங்கின்ற குழப்பம். So, கடைசியாக சரி முதலில் செய்வோம், பின்பு ஆராய்ந்து கொள்ளலாம் என்ற எண்ணத்தில் இறங்கி விட்டேன். இது ரத்தப் புற்று நோய் பற்றி நான் உருவாக்கும் ஒரு கையேடு. இந்த முயற்ச்சியின் intended objective என்று சொல்ல வேண்டுமானால் ஒருவருக்கோ இல்லை ஒருவரின் சுற்றத்தாருக்கோ ரத்த புற்று நோய் உள்ளது என்று அறியபட்டால் அதன் தொடர்ச்சியாய் வரும் நிகழ்வுகளை எதிர் கொள்வதற்கும், உடனடியாக எடுக்க வேண்டிய தீர்மானங்களை எடுப்பதற்க்கும் உதவும் ஒரு சிறு கருவியாக தான் நான் இந்த கையேட்டின் பயனை பார்கிறேன். முதல் முயற்சியாய் நான் American Cancer society உருவாக்கியுள்ள Handbook on Acute Myeloid Leukemia என்ற புத்தகத்தை தமிழாக்கம்(இதை தமிழாக்கம் என்று சொல்வது நியாயமான வார்த்தை அல்ல, for want of a better word, am using this என்று கொள்ளலாம்.) செய்ய முயல்கிறேன். இந்த முயற்சியில் நான் மருத்துவ சொற்களுக்கு உரிய தமிழ் வார்தைகளையெல்லாம் பயன் படுத்தவில்லை, பயன் படுத்த முயற்சி செய்யவும் இல்லை. இது ஒரு authentic medical materialம் இல்லை. முன் சொன்னதை போல வாழ்கையில் diagnosis of cancer for oneself or for a loved one, எங்கின்ற ஒரு தருணத்தை சந்திப்பவர்களுக்கு, அதை ஒரளவுக்கு சரியாக அணுக பயன் அளிக்கும் என்று நான் உணர்ந்த விஷயங்களின் தொகுப்பு தான் இந்த கையேடு.

இன்னும் தெளிவாக சொல்ல வேண்டும் என்றால், நான் என் வாழ்கையில் அத்தகைய ஒரு தருணத்தை சந்தித்த பொழுது, இந்த ஒரு information or a perspective, if it is available would be very nice என்று feel பண்ணி, that it was missing என்று உணர்ந்த சில விஷயங்களை, gapsஐ fill பண்ணும் ஒரு முயற்சி இது. அத்தகைய ஒரு முயற்சிக்கு இந்த ACS material ஒரு நல்ல frameworkஐ அளிக்கிறது என்பதால் அதை ஒரு அஸ்திவாரமாக வைத்து கொண்டு இதை ஆரம்பிக்கிறேன்.

முதலில் Cancer என்றால் என்ன என்று பார்போம். நம் உடலில் உள்ள ஏதாவது ஒரு பாகத்தில் உயிரணுக்கள் கட்டுக்கடங்காமல் வளர ஆரம்பித்தால் அதுவே புற்று நோய் வளர்ச்சியின் அறிகுறியாகிறது. புற்று நோயென்பது பல வகைகளாக இருக்கும் பொழுதிலும் இந்த உயிரணுக்களின் கட்டுக்கடங்காத வளர்ச்சியே புற்று நோயின் மூலமாக அமைகிறது.

உடலில் உள்ள சாதாரண உயிர் அணுக்கள் வளர்ந்து, பிரிந்து(process of Cell Division) ஒரு ஒழுங்கான முறையில் மறைந்து விடும். மனிதர்களின் குழந்தை பிராயத்தில் சாதாரண உயிர் அணுக்கள் மிக தீவிரமாக பிரிந்து நாம் இளம்பிறாயத்தை அடையும் வரை நம் உடலின் துரித வளர்சிக்கு வழி வகுக்கிறது. அதன் பிறகு ஒரு ஸ்திர நிலையை அடைந்து ஏதோ ஒரு தேவையின் பொருட்டு மட்டுமே பிரிகிறது. அந்த தேவை வேறொரு உயிரணுவின் மறைவை சரிப்படுத்துவதற்கோ, அதன் சிதைவை சரிப்படுத்துவதற்கோ ஏற்படுவதாக இருக்கும். ஆனால் இந்த புற்று நோயணுக்கள், அப்படி அல்லாமல் கட்டுக்கடங்காமல் பிரிந்தும், சாதாரண உயிரணுக்களை ஆயுட்காலத்தை தாண்டி வாழ்ந்தும் சாதாரண உயிரணுக்களில்ருந்து மாறுபடுகிறது.

எல்லா உயிரணுக்களிலும் DNA என்ற ஒரு சமாச்சாரம், அந்த உயிரணுவின் செயல்பாட்டை நிர்ணயிக்கின்றது. இந்த DNAவுக்கு சிதைவு ஏற்படும் தருணங்களில் இந்த Cancer cells உயிர்பிக்கிறது. பல நேரங்களில் இந்த சிதைவை அணுக்கள் தானே சரி செய்து கொண்டு விடுகின்றது, அப்படி இயலாத தருணங்களில் அவை Cancer cellஆக மாறுகின்றன. இந்த சிதைந்த DNAக்களை நம் வம்சாவளியாகவோ, நம் சுற்றுபுறத்தில் ஏற்படும் ஏதோ ஒன்றோ உண்டாக்குகிறது. புகை பிடித்தல், மற்றும் தொழிற்சாலைகள் உமிழும் சில விஷபுகைகளை சுவாசிப்பதும் சில உதாரண காரணங்கள்.

பெரும்பாலும் கேன்சர் ஒரு கட்டி வடிவத்தில் தான் உருவாகிறது. ரத்த புற்று நோய் போன்ற சில கேன்சர்கள் இதற்கு விலக்கு. அதே போல உருவாகும் எல்லா கட்டிகளும் கேன்சர் அல்ல. அத்தகைய கட்டிகளை beningn tumor என்றும் கேன்சர் கட்டிகளை malignant tumor என்றும் சொல்வர்.

இந்த கேன்சரின் இன்னொரு முக்கிய பிரச்ச்னை அவை தான் தோன்றிய இடத்திலிருந்து கிளம்பி உடலின் பல பாகங்களுக்கும் பரவ ஆரம்பிக்கும். மேல் சொன்ன சில சிறு குறிப்புகள் மட்டுமே Cancer பற்றி பொத்தாம் பொதுவாக சொல்ல கூடிய கருத்துக்கள். மற்றபடி Cancer குறித்து எழும் பல கேள்விகள், அதை குணப்படுத்த முடியுமா, அது உயிர் கொல்லியா, அதனால் நோயாளியின் வாழ்க்கை எப்படி எல்லாம் பாதிக்கப்படும் போன்ற பல கேள்விகளுக்கு அவை எந்த வகை கேன்சர், அந்த கேன்சரின் நிலை என்ன போன்ற பல factorsஐ ஒத்தே பதில் அமையும். அத்தகைய இந்த பல வகை கேன்சர்களில் ஒன்று தான் இந்த Leukemia எங்கின்ற ரத்த புற்று நோய்.

இந்த கையேட்டில் நாம் Acute Myeloid Leukemia(more specifically Adult Acute Myeloid Leukemia, AML) என்ற கேன்சர் பற்றிய சில விஷயங்களை முதலில் அறிந்து கொள்வோம

சென்ற வாரம் நடந்த நிகழ்ச்சிகளில் மனதையும் சிந்தனையையும் அதிகம் பாதித்த ஒரு சம்பவம் Mehta தம்பதியினரின் வழக்கு. 26 வாரமாக வயிற்றில் இருக்குசென்ற வாரம் நடந்த நிகழ்ச்சிகளில் மனதையும் சிந்தனையையும் அதிகம் பாதித்த ஒரு சம்பவம் Mehta தம்பதியினரின் வழக்கு. 26 வாரமாக வயிற்றில் இருக்கும் அந்த சிசு, அது பிறந்தால் ஒரு தீவிர இருதய நோயுடன் பிறக்கும் என்று pre-natal diagnosis முடிவுகள் மூலம் அறிவதால், அது நோயுடன் பிறந்து உடல் வருந்துவதும், அது வருந்துவதை பார்த்து அதன் சுற்றங்கல் வருந்துவது, இதை எல்லாவற்றையும் விட அந்த சிசுவின் உயிரை (to call the unborn foetus as life itself is being debated, hence to equate abort to killing the life is even more contentious) இன்றே அழித்து விடுவது உத்தமம் என்று அதன் தாயும் தந்தையும் எடுத்த முடிவும் அந்த முடிவுக்கு உள்ள சட்ட சிக்கல்களை எதிர் கொள்ள தொடுக்க பட்ட ஒரு வழக்கு அது.

இந்த வழக்கு பற்றி மீடியாவில் அதனோடு தொடர்பென உணரப்பட்ட பல விஷயங்கள் அலசி ஆராயப்பட்டன. அவைகளில் சில குறித்த என் எண்ணங்களை இங்கே பகிர்ந்து கொள்கிறேன். Pre-natal diagnostic techniques என் கின்ற ஒரு விஷயம் a peek into the life of the unborn என் கின்ற அளவிலும் வயிற்றில் வளரும் குழந்தை ஆரோக்கியத்துடன் வளர்கிறதா என்ற அறிதலின் curiosity காரணமாகவும் it has been given a kind of importance that is probably more than what it is worth என்ற ஒரு சாராரின் கருத்து உள்ளது. Pre-natal diagnosis is a complex process and some of the initial tests which most pregnant women are made to undergo these days can be grossly inaccurate. On most instances requires a series of follow up examinations and rounds of expert opinions to confirm a possible abnormality. Unfortunately interventional therapies at that stage to remedy an abnormality suspected/confirmed is not very evolved at this point in time. In such a scenario it must be admitted that the fundamental aim of pre-natal diagnosis is to prevent the birth of an abnormal child. Ofcourse no one can contest that the parents who have choosen to bring the life to this world has their best interest in it and must have the fullest right to do whatever they want to do in a situation when a possible abnormality gets suspected. But for that decision they should also keep this perspective that a child with an abnormality should be as welcome to this world as a normal child.

Am not trying to romanticise it, ஒரு குறைபாடுள்ள குழந்தையை வளர்பதில் உள்ள அத்தனை சிரமங்களையும், அதன் வலிகளையும் உணர்ந்து சொல்லப்படும் ஒரு வார்த்தை. I write this post to register my strong disagreement with the actions of the Mehta's in seeking to abort the child.

Serial and Serious killing

I am one of the few lucky people to hold a futuristic device. The device can zoom onto any place in the city and pick up the audio signals as well.It is with help of a advanced satellite technology that i could be able to capture events in the city. One of the series of events i am to describe is the aftermath of people's reaction to the killing.

It seems a group of people are on the move killing people by striking thier heads with stones and burning them. It in some sense seems to resemble the period of stone age and the time when man started flint stones. The combinaton of these two is wreaking havoc in people's minds and triggering of unrest in people' physchology. More than the killers needing physchatric treatment the public seems to be needing the treatment.
One such event i was able to witness was on a busy shop next to a highway.The loudspeaker was blaring the song which i translate that into english.
"fear is stupidity, fearlessness is dravidian property( acham enbathu madamai anchamai dravidian udamai)". The group of people seem to completely ignoring this wonderful song. This song had in the past kindled the fire of action among people and triggered them into actions befitting a karmi yogi. Oppose hindi and seek not the fruits of the opposition was the effect of this song. Such was the powerful effects of this song in the past. Hovewer the song in this present times couldnt have any effect on the people.On hearing this song i myself felt a new energy surging into my physche.But among the public a strange fear still lurked. It made them wonder whether the physchos might have planted this song among thier midst either to mock them or make them fools. Groups of women who were interviewed by TV channels seem to howl about the killers. The target of the attack were nightwatchmen and thier wives howled out thier fears. The watchmen who were brave during normal times refused to do night shifts.A strange phenomenon witnessed was that earlier dogs were beaten and hounded out.The poor creatures on the dog , straying on the roads were beaten mercilessly and driven away were now treated with biscuits,bread and milk. It made me wonder the oppurtunistic tendencies among the people.WIse men say that "small rusted peice may be used for cleaning the teeth"(siru thirumbum palluku uthavum), i.e usefullness of even small things will be in a big way. The people who had treated the stray dogs inhumanly in the past nowadays treated them with purposefulness, note that it was not human kindness but only for the sake of the stray dog's purposefullness. It was belived that pack of stray dogs could ward off the killers.
The other aspect was the resourcefullness of band of young men brandishing logs of wood in the night and roaming in the nights hoping to catch the killers. They could not go out singly for fear of killer or killers taking them out, but gathered courage only to roam around as mobs thus justifying that only mob rule and mob physchology prevailed in this part of the country as is in any part of the country.On 2 occasions the group of young men had gone overboard with thier enthusiastic energetic spirits had beaten a butcher in the morning. the butcher had it seems done his job in cutting down a goat and had his hands stained with goats blood . THe group had mistakem him to be a killer and in a frenzied mood mistook him to be the serial killer. On another occasion a resident of a locality had been mistaken to be a serial killer. The law came in between the hunt of these people and advice the young men to stop such tactics as it proved futile.Now the the men are nerve strung and stressed out.
As for the law the men are patrolling with minumum protection. While those in power are with unneeded and overdose of security the law enforcing people are patrolling with minimum weapons. It is such people who need the required weapons to tackle the gang of serial killers. Today a news came out that it was a gang operating such killings for monetary gains, but the puzzle is why the gang burns the victims.The law enforcers claim that its not physchos handiwork but only a gangs work.
Public's murmur can be heard here and there that it looks more like a masking effect and that real culprits are not caught.
A strange phenomenon is a shower of stones being pelted on hutments of people by a person or group of persons. ON lookout after the incident the person or persons are not at the scene of the crime. This has made the women stressed out and looks like they need physchatric counselling.The men thought put on a brave face still seem to have fears lurking within them.
Suddenly everybody has turned into a sherlock holmes .everybody suspects the rest and this interesting problem is apt problem to be posed to a mathematician.What is the probabality that a given person will be correctly judged to be a killer given that the opinions and thoughts of the people is BIASED.
Who will be the next victim? Who will catch the killer? Will the killer turn out to be a normal person or a physcho?These are thoughts running in the otherwise idle people's minds.The writer feels that a serial and a TV show can be hosted .The writer is sure that people will flock to see this serial or show, as thier is an element of suspicion and intrigue in the air.
The writer had first hand experience with a confused lad. IN a locality the writer interviewed a lad about the killings.THe lad pointed out some places where the killings had taken place. The lad was telling the truth but had an element of exaggeration and element of fear in him.To the writer there seemed a mathematical pattern in the spots in where the killers had done away with the victims.Could the killers know mathematics as well?

The actor had just got out of his car. he was puffed up and laced with lots of make up.he was around 55 but looked 25 owing to his cosmetic makeup. the fool was a die hard fan of the actor and raised slogans shouting praises of his idol.
The lives of the fool and the actor were inter dependant. the fool was paid 50 rupees per day while the actor was paid crores per day for his histronics.from politicians to policemen the actor was praised for all his gimmicks. the actor was swarmed by groups of college students and girls who tore at his shirt once he came out of his mercedes benz.
The fool tried to get close to his idol but couldnt reach the actor.the fool's dream to touch his idol was a continuing issue.the actor waved his hands and felt his pumped out ego even growing more. the actor at many interviews had stated that owing to his fans he had grown. not a single person had known the the actor had himself been a fool who had his own idol 20 years back. he had in the same manner tried to reach out to his idol 20 years back but each time had to return empty handed. many people at present had been lucky in touching the actor and a few had been more lucky in recieving flower petals from the actor himself.
The fool had a single meal a day while the actor was pampered in star hotels and grown fat while the fools stomach were carved inside owing to lack of food. .the fool though knowing that his and many people's money had flown in the actor's coffers had created this fat actor ,still insisted on following the actor on his trips outside in the city during shootings.
Many people had tried to follow the actor while shootings were taking place. People had grown mad about this particular actor.
They had lost thier sense of identity and individual thinking and whatever the actor was saying the people took to heart and followed it to hearts content.
While many in the crowd held the actor in a high pedestal a few were jealous that the actor was enjoying an unparalleled life style. Even political parties were wary of this particular actors popularity. though he looked faded in appearance he was considered an adonis and a dream boy and not a person bursting in his seams. He was a grand father at this time and still posed as if he was a youthful person.
All the actors and actresses poured praises on this actor fearing that thier careers would be affected if they didnt do it otherwise. A wise man was watching all this paraphenilia and felt that not only the fool was fooled but enmasse people were fooled by this actor.
Not only the fool was foolish but enmasse people were ending up in being fools in falling for this actors histronics.The actor was finally a person who like other persons eat ,slept and had the mudane feelings of others. It made the wise person think of the frailities and foolish imaginations of the public in holding this actor in such a high pedestal which he didnt deserve at all.
Had the person who was an actor won a nobel prize , the wise person himself would have held the actor in high esteem. To his perspective the actor was after all doing his job as other people were carrying thier jobs.
The hard earned money of all the people were ending up in the actor;s coffers after all for just 3 hours of so called entertainment.
It made the wise man wonder was thier a single fool or a pack of fools?

Where will IIT' get their faculties from?

Link to a blog post. I find the numbers talked about in Jalote's article a bit difficult to understand?. Some basic information I know of faculty recruitements for IIT's is a pre-requiite of 3 yrs of post-doc experience and such, so where comes the question of Phd's passing out in the next 2-3yrs put in the supply category. Also would the new emerging IIT' be comprised of totally entry level faculties. And what about the huge resource pool of Phd's who have passed out in the last 5-10yr, still moving from one post-doc to other and would very much like to be considered for a position in IIT's. I hope this strange looking numbers and arguments is not just yet another instance of reluctance on the part of IIT's to scale up. Am forwarding this blog post to a few people, who might want to respond.

இந்த contextல் பார்க்கும் பொழுது சுஹாசினியின்-- ஹாசினி பேசும் படம் நிகழ்ச்சி really comes as a breath of fresh air. சினிமாவை வெறும் ஒரு போகப் பொருளாக அணுகாமல் அது ஒரு கலை வடிவம் அதை விமர்சனம் செய்வதற்கு முதலில் அதை appreciate பண்ண தெரியணும் அந்த appreciationகு ஒரு பயிற்சி தேவை என் கின்ற அணுகுமுறையில் படங்களை அணுகும் ஒரு நிகழ்ச்சி. பொதுவாக கொடுத்த காசுக்கு சிரிச்சோமா, ஏதோ மசால் வடை போல சாப்பிட்டோம் பிடிச்சுதா பிடிக்கலையா எங்கின்ற அளவிலேயே இருக்கிறது சினிமா குறித்த அணுகுமுறை . இது எனக்கு ஒரு விதத்தில் ஆச்சிரியதையும் வருத்தத்தையும் தருவதற்கு காரணம் சினிமா அளவுக்கு மக்களை engage பண்ற இன்னொரு விஷயமான cricketகு அந்த அளவுக்கு ஒரு ரசிக்கற ஒரு உணர்வு இருக்கிறது. Ofcourse, it is a irrelevant comparison, but still i hope some people can relate to my ஏக்கம் and understand what i mean here.

She basically starts the program by presenting a short overview of the genre in which the movie belongs to. Then she refers 1-2 main films in this genre and says the good and bad things of those. This puts the film and how one should evaluate the film by giving us a context. She typically takes up 1 or 2 films for review in one episode. She follows up a simple intro to the film with a interview of the people involved in the film. She tries to get their perspective on some of the pivotals items of the film. Infact, she also goes further and tries to talk a bit about background of the director and how the trajectory of the directors life has impacted the film and such things. என்னை பொருத்த வரை இது ஒரு ரொம்ப ஆக்கபூர்வமான நிகழ்ச்சி. She also tries to give a lot of importance and visibility to small and medium buget films and their creators.

குறைகள் அப்படினு சொல்லனும்னா நிரைய்யா she repeats herself in the show. It is so blatant, almost பாதி programme சொன்னதையே திருப்பி சொல்லிண்டிருகற உணர்வு ஏற்படறது. And Suhasini often tells the answer to her questions to the interview and puts it like..this is my question and this is your answer right kinda conversation. She is not a good interviewer. Probably she can watch her cousin Anu Haasan program to better herself on that front.

Another thing is her costumes for the show. Ofcourse this is a very personal thing. எனக்கு பிடிச்ச Suhasini அப்படின்னா ஒரு conventional Saree, or a Churidhar, where the care for her dressing does not appear very transparent. For people who had their growing years in 90's major impact of Suhasini was like Sindhu Bhairavi(I identify that character a lot with Raji of இரும்புக் குதிரைகள்..) and thats the Suhasini one wants to see.

The Aarushi Talwar murder trial has been ripped apart by the media so badly that there is any new story that comes out of it, is not a perspective/angle that was not thought of or discussed by the media already.

Given the larger public interest in this story, it is understandable that the news media has gathered to get their "first byte" and report as breaking news when Dr. Talwar was released on Saturday. But when the Talwar family is begging the media to just let them have some moment of private time to grieve,and that they would address the media at a later time, it is extremely outrageous that some section of the media is chasing them till they get into the house.

In such stories with great viewer interest(for multi-various reasons) it is difficult to blame the media alone on sensationalism, utter disregard to individual human emotions and such. Because there are various instances when even the involved people have made use of the media to convey a message of theirs, gather public support and for other reasons. But in spite of all this it is sad that the media persons gathered around on Saturday does not respect such a basic request of the family.

The media might in turn point the fingers at the viewers for their interest and blame that as reason for their actions. So I feel as a responsible viewer, we can register our protest in a small way to such actions by the media by either switching off the television to register our refusal to watch it on such instances or change to another channel. I think this is the barest minimum thing that we should do.

The earthquake had struck chennai again after quite a long time. People were stricken with fear and were wondering the cause of this mild tremor.

I was watching some movie and had a power cut around 5.00 pm. I didnt know about this tremor as i had become thick skinned. Some time back i had commented to a friend of mine that people here had exceeded thier bad karmas some fury of nature would strike its hands on them to remind them of the merits of good karma. I felt that my words were becoming truer.

As i walked by the streets people were huddled together and whispering among themselves about this tremor.I got the information from them that a tremor had struck chennai.Some people attributed this as fury of GOD as people's relegious beliefs had dwindled down.While the non belivers who felt that its was thier supreme right to not believe in GOD relying in the scientists reasoning. Among the non-believers were those who inwardly believed in GOD but put up a face that they didnt believe in GOD. Among those who believed in GOD , they poured ghee,milk , burned incense sticks and chanted vedas hoping that a major quake could be averted.
The political parties workers felt that the opposition party had planted a huge device underground which could cause this tremor so as to de-stabilise the ruling regime. While the opposition party raised thier voices saying that unplanned draining of ground water could have resulted in this. Among the believers in GOD in opposition they felt that the leader of opposition was not given her right place so this could have resulted in the tremor.
The common man who by this time had got used to the common ramblings of the various groups felt his life at stake and a fear crept inside him. The next quake and he will faint.
For those who felt that brahmins were weak kneed, this tremor had shown that indeed all were weak kneed people. The papers potrayed how people with fears in thier faces looked at each other in thier faces., This indeed makes true the statement "kettle calling the pot black".
I felt before we make allegations at other group of people whether we are really morally right.

One more major earthquake in chennai and the true bravery of people will come to the streets.Its ironic that people pride in thier false bravery.

This post is about the news item on the recommendation of cholesterol drugs for children as young as 8yrs old to treat obesity and risks associated with it.

It is disturbing to read about the advocacy of medication to deal with childhood obesity and the associated risks. This needs to be exercised as an option only in very specific cases after a very careful evaluation. On most other instances one should seriously try life style modification options to get the desired health benefit in such young age groups.

Childhood obesity is rising at alarming levels in India and strong efforts needs be done to address this issue without causing long term health hazards to the children. Schools should consider having something like the health card(aligned to the progress report card) and can offer reward points for adopting to healthy lifestyle choices. A child can be rewarded with a few good points if it brings vegetables for lunch and avoids/limits junk food for the snack box that the children bring to school.

I came across an online competition from the Hopelab which uses innovative interactive game techniques to improve health and quality of life to children Ruckus on getting innovative ideas to get the kids moving and driving them to active lifestyle. We could also evolve ideas that would work in our context and try its effectiveness.

Wanted to document my thoughts on something I read recently in The Hindu and that which set me to think on a few related things.

A chennai doctor has opined that sensitizing the public on cadaver transplantation for organs like kidney would greatly address the large imbalance
in the demand and supply(Ah..well, thats the kind of language they speak) of kidney for the patients requiring transplantation. This is not just his personal opinion
but a view shared by many. He also adds that such an initiative has paid off in the case of eye donations. I think there is a distinct difference between the two scenarios which relates to the issue of "brain" dead and the "real dead" dead, and this difference is what makes this suggestion extremely scary to me.

Some basic information on this (whole write-up is described in the Indian context).

Treatment options for the end-stage kidney disease are life-long dialysis and kidney transplantation. Dialysis is not a permanent solution and may become medically unviable to continue after some time. Kidney transplantation is recommended for end-stage renal failure as the patient can get back to near normal life style after the surgery and post- operative care. Kidney transplantation can be acquired from a live or a cadaver donor. In India majority of the kidney transplantation is
a live donor transplantation, where a near relative donates
one of their kidneys to their dear ones. Organ transplantations from a cadaver constitutes a very small percentage.

Cadaver transplantation is a process where healthy live organs from patients who are declared "brain dead " are retrieved and transplanted in patients with end-stage disease. The eye donations (which I personally do strongly advocate and has materialized convincing my own family in the case of two of my grand parents) are distinctly different in the sense, they are taken from a “dead dead” person within a short period of time after the actual clinical death of the person. But most other organ donations like kidney for organ transplantation is done in the setting of brain death.

Let me just summarize some of my understanding on this issue of brain death focusing on this specific context of organ transplantation. Traditionally, death has been defined as the cessation of all body functions, including respiration and heartbeat. It became possible to revive some people after a period without respiration, heart beat, or other visible signs of life by maintaining cardiovascular and pulmonary functions of the body through modern medical technology. But these technologies indiscriminatingly sustain such bodily functions on people who are considered by the medical community as having irreversibly lost all brain functions and also of those who are critically ill patients. With modern medical technology blurring an important distinction between those who are dead(read "brain" dead ), those
who are dying and those recovering from a life saving surgery or severe trauma- defining death with better criteria became an important issue. One of the reasons for the update of definition of death is this issue of obtaining viable organs from neomorts for transplantation.

This definition of brain death and the consequences of it are a highly contentious issue. Independent of the transplant interest to this definition this usually raises heated debates on various counts such as its consequences to the question of continuing life support to the brain dead. Over the world when issues of Euthanasia, mercy killing questions props up it is argued and debated with strong emotions by the rationalists, religious bodies and the larger public.

Though sort of digression to the topic in discussion, I have an intense unease towards the arguments and rationale put forth against continual of life support to the patients who are perceived by the medical community as “brain dead”.
I personally had to deal with this situation twice in my own life to argue to continue with life support when it was perceived by the medical community as a futile exercise. In modern medical decision making, personal choice is upheld as the highest good(I have some disagreement even on this, but that is something that will be hard to contest through reasoned arguments). On instances when that was not feasible (patient is not in a state to make a decision and does not have a will expressed before), immediate family is put into an unenviable position of having to decide on behalf of the patient. The arguments in favor of pressuring the family ranges from limited medical resources being put into effective use to have to evaluate the “quality of life” of the dying individual. I completely fail to understand what is this quality of life that these people talking of?. Does not ones life have a meaning if he/she cannot experience the sensory pleasures and the life needs to be rendered futile. Does not ones life serve a purpose for the simple fact that the satisfaction it brings to their immediate family that the person is still surviving (this becomes all the more relevant in instances when the life in question is too young to die)? In a normal instance everyone does value quality of life as prime is accepted, but in the end of life process, quality of life is often surrendered early on. And the argument on putting resources to effective use is not so much about bed shortages in the hospital or a case by case evaluation of the hour and money spent on some one who has a better chance of survival than the other.

Getting back to the defining death(brain dead) for the purpose of organ transplantation, I think it is a legitimate fear that a dying person would be less aggressively cared for if that person is perceived by the doctor to be a potential source of organs. I don’t think that these concerns are unfounded and can be ignored as something that does not warrant any serious thought. Given the crass commercialization and the consequent compromise on the ethical/moral standards of the medical professionals this is a serious issue. The people who have carried out surgeries on unwilling/un-informed donors or those who have been bought for money are not some quacks or crooks, but actual doctors trained at some of the best colleges in India and abroad and are recognized by the Indian Medical Association(IMA)

In the current climate of general public distrust of medical professionals, if public confidence in such transplantation programs is to be established, a high level of scrutiny by the government and a body of well meaning medical professionals must first be established before they form legislation and sensitization of public towards such transplantations. Otherwise it would be like putting the cart before the horse. The very thought of our loved ones and other lives at ICU, would be evaluated with cost benefit analysis of its survival against odds vs hastening death for organ harvesting is too scary to say the least

இனி தமிழ் சினிமாவின் பல கதாசிரியர்கள் தங்கள் கதைக்கான பாத்திரங்களை வடிவமைக்கும் பொழுது "ச்சே இந்த role செய்வதற்கு ரகுவரன் இல்லாமல் போய் விட்டாரே " என்றும் சினிமா பார்க்கும் ரசிகர்கள் "இந்த role ரகுவரன் செய்திருந்தால் எப்படி இருந்திருக்கும்" என்றும் பல முறை அங்கலாய்பார்கள். ரகுவரனின் மரணம் தமிழ் சினிமாவுக்கு ஒரு பேரிழப்பு.

ரகுவரனின் முதல் படத்தை இயக்கிய ஹரிஹரன் அவர்களுடன் எனக்கு நல்ல பழக்கம் உண்டு(என் அண்ணனுக்கு அவர் ஒரு வகையில் குரு மாதிரி). அவர் ரகுவரன் அறிமுகமான காலத்தில் இந்திய அளவில் Amitabh Bachanன் Angry Young Man idea ஆக்ரமித்திருந்த காலம், அதை தமிழகத்திலும் பிரதிபலிக்க கூடிய ஒரு முகமும் திறமையும் ரகுவரனிடம் பார்த்தேன் என்று குறிப்பிட்டார்.

வில்லத்தனம் என்றால் கொடூரமான முக அசைவுகள் மற்றும் செய்கைகள் என்ற நிலையை மாற்றி Sathyaraj create செய்த ஒரு acting style வழியில் தனக்கென தனி முத்திரை பதித்தவர் ரகுவரன். வில்லனாகவும் மட்டும் இன்றி பல குணச்சித்திர பாத்திரங்களிளும் தன் performance மூலம் தன் தனித்துவத்தை நிலை நாட்டினார்.

போதை பழக்கத்துக்கு அடிமையானதும் அதன் விளைவால் ஏற்பட்ட உடல் நலிவுமே அவரின் அகால மரணித்திற்க்கு காரணமாக சொல்ல படுகிறது. அகால மரணம் என்பது மிக கொடுமையான ஒரு நிகழ்வு. அந்த கொடும் நிகழ்வுக்கு இத்தகைய பழக்க விளைவுகள் ஒரு வகையில் காரணம் என்னும் பொழுது இந்த கொடுமைக்கு அவரையே கூட ஒரு கர்த்தாவாக எண்ணத் தோன்றுகிறது. அவரின் 10 வயது மகன் கதறி அழும் காட்சியை பார்க்கும் பொழுது "No pleasure in life is worth, if you think it would even remotely be a cause for such a pain" என்று தான் எண்ணத் தோன்றியது.

Shivani Sud- Junior Noble for Cancer research

Shivani Sud, a 17-year-old girl from Durham (North Carolina), submitted a bioinformatics and genomics project to Intel Science Talent Search that focused on identifying stage II colon cancer patients at high risk for recurrence and the best therapeutic agents for treating their tumors.

Shivani developed an interest in cancer research after an immediate family member diagnosed with brain tumor was saved by the doctors.

This is what the Society for Science & the Public (organizers of the competition since its inception in 1942) had to say about Shivani’s work:

The standard method of characterizing tumors relies on visual information, including size, degree of metastasis and microscopic structure. Shivani’s 50-gene model for predicting the recurrence of colon cancer instead uses gene expression profiles to link multiple genetic events that characterize various tumor types. She created her model using two public data sets containing 125 patient samples and coupled it with clinical data to plot statistically significant survival curves. She then used her model to identify drugs that may be effective in treating stage II colon cancer.

எனக்கு தெரிந்தவரை இந்த அளவுக்கு ஒரு cult following..superstar status popularity உள்ள தமிழ் எழுத்தாளர் வேறு யாரும் இல்லை(ஒரளவுக்கு பாலகுமாரன் இந்த categorieல் உள்ளார் என்றாலும்... சுஜாதாவுக்கு தனி இடம்).

எங்களை போன்று சிற்றூர்களிலும் கிராமங்களிலும் internet, satellite television இல்லாத காலத்தில் தமிழ்நாட்டில் வளர்ந்த பல இளைஞர்கள் வாழ்கையில் சுஜாதாவின் கதைகளுக்கும், கட்டுரைகளுக்கும் பெரும் பங்குண்டு. பின்னாளில் வேறு பல எழுத்துகளுக்கும் சிந்தனைகளுக்கும் அறிமுகம் ஆன பின்பு சுஜாதா எழுத்துகள் மீது மாற்று, எதிர் கருத்துக்கள் இருந்தது என்ற போதிலும் வாசிப்பின் இன்பத்தை எனக்கு வழங்கியவர்களில் R. K. Narayanக்கும் சுஜாதாவுக்கும் தான் முதலிடம்.

தனது 70வது வயதில் கற்றதும் பெற்றதும் தொடரில் வயோதிகம் பற்றி, வாழ்கையில் pilot ஆக வேண்டும், சாதிக்க வேண்டும் என்ற கனவெல்லாம் மாறி காலை எழுந்தவுடன் ஒழுங்காக பாத்ரூம் போனாலே நிம்மதி என்ற நிலை..என்று பொருள்பட கூறி..வாழ்கையே ஒரு வித progressive compromises than என்று சொல்லி இருப்பார். இந்த மாதிரி அவர் எழுத்தில் பசுமையாய் நினைவில் பதிந்த விஷயங்கள் பல பல..

சுஜாதாவை இரு முறை நேரில் பார்த்திருகிறேன். அலைபாயுதே படத்தின் preview காட்சி பார்த்து விட்டு வெளியே வந்த அவரிடம் எங்களை(நானும் என் அண்ணனும்) அறிமுகப்படுத்திகொண்டு தங்களின் தீவிர ரசிகர்கள் என்றும், தங்களை எப்போதாவது ஒரு முறை சந்தித்து தங்கள் எழுத்துக்களால் எங்களின் மீது நீங்கள் ஏற்படுத்திய தாக்கத்தை பகிர்ந்து கொள்ள வேண்டும் என்று சொன்னோம். வீட்டுக்கு phone செய்து விட்டு வாருங்கள் என்று சொல்லி விட்டு கிளம்பி சென்றார். அவர் நமக்கு வழங்கிய இன்பமான தருணங்களுக்கும் கற்ற விஷயங்களுக்கும் தனியே சந்தித்து நன்றி சொல்லி விட வேண்டும் என்று கூறி கொண்டோம்.

அசோகமித்திரன் அவர்களின் பவள விழா நிகழ்ச்சியில் கிழக்கு பதிப்பகம் Director Sathya Narayananனுடன் பேசிக்கொண்டிருந்த பொழுது அவர் அசோகமித்திரன் ஒரு one of the earliest bloggers என்று அவர் எழுத்தை பற்றி குறிப்பிட்டார். அப்பொழுது அதன் அர்த்தம் சரியாக விளங்கவில்லை. பத்ரி அவர்கள் சுஜாதா அஞ்சலி பதிப்பில் அசோகமித்திரனும், சுஜாதாவும் அவர்கள் கண்டது, கேட்டது, தங்களை பாதித்த விஷயங்கள் பகிர்ந்து கொள்ளும் விதத்தில் Bloggers என்று குறிப்பிட்டு எழுதிய பொழுது அர்த்தம் விளங்கியது. சுஜாதாவை பற்றிய இந்த observation ரொம்பவும் முக்கியமான ஒன்றாக நினைக்கிறேன். இதுவே அவர் வாசகர்களிடம் அவருக்கு ஒரு அன்னியோன்யத்தை ஏற்படுத்தியது என்று நினைகிறேன்.

மரணம், அந்த பிரிவின் தாக்கம் என்கின்ற உணர்வுகளை பல பரிமாணங்களில் கடந்த 5 வருடங்களாக அனுபவித்து வரும் எனக்கு, சுஜாதாவின் மரணம், அதன் தாக்கத்தின் எனக்கு உள்ள தொடர்பு பற்றி குறிப்பாக சொல்ல ஒன்றும் இல்லை. சமீபத்தில் அவர் வாயாலேயே அதை பற்றி சொன்ன ஒரு குறிப்பு தான் நினைவுக்கு வருகிறது.

Observations in the Bangalore-Chennai Rail Journey

Just wanted to record my thoughts on 2 things that one commonly observes in the train journey between Chennai-Bangalore in a general compartment day time travel. One is the nuisance by the transgender community and the other is about the vendors selling fruits, vegetables and other assorted items like toys, books and stuffs.

This nuisance by the transgender community has increased alarmingly in proportion. It has almost become a definite thing to encounter a Hijra twice (once leaving Chennai and once while entering Bangalore). They ask for money and when the person (usually young males are the target) refuses to give them money, they turn quite abusive. They literally demand that they are given at-least 10Rs. The police or the railway authorities do not seem to have any control over these peoples entry into the train. Once, I was even told by the ticket examiner that even when he interferes they can turn nasty at them.

If the transgender community wants to gain social acceptability, they as a community needs to do something about this.

As regards vendors most of the commuters welcome their presence. People get to buy fruits and vegetables of the local produce at a much cheaper price than what they would pay in Chennai or Bangalore. As they do not sell fresh ready to eat food their business does not pose any competition to the Indian Railways Catering (IRCTC) as well. But, unfortunately these people are harassed as illegal travelers. Instead, these people can be given licenses making their sales an approved one. Infact, the government can offer such licenses on priority to physically handicapped individuals, under-privileged people and members of some self-help women group and members who has been in service on the route for a long period of time.

கவலைப்படாதே சகோதரா! -- புத்தக விமர்சனம்

இந்த புத்தகத்தின் விமர்சனம் எழுதும் எண்ணத்தில் தான் இந்த பதிவை எழுத ஆரம்பித்தேன். ஆனால் இந்த புத்தகத்தை பற்றிய நான் பகிர்ந்து கொள்ள விரும்பிய விஷயங்கள் at a more generic level புற்று நோய் தொடர்பான மாற்று சிகிச்சை முறைகள் பற்றிய கருத்துகலொடு ஒத்து போவதால் நான் இந்த பதிவை just அந்த புத்தகத்தை ஒற்றியதாக குறிக்கி கொள்ளவில்லை.

Cancer என்பது ஒருமுக நோய் அல்ல. அது பன்முக, பலவேறு பரிமாணங்கள் கொண்ட ஒரு நோய். உதாரணத்திற்கு ஒருவருக்கு ரத்த புற்று நோய் உள்ளது என்று மட்டும் சொல்வது, அந்த நோயின் தாக்கத்தை பற்றியோ, அது எந்த அளவிற்கு அவரின் உயிருக்கு அபாயகரமானது என்றோ அறிந்து கொள்ள உதவாது. ரத்த புற்று நோயில் எந்த வகையை சார்ந்தது, அதன் தாக்கம் அல்லது வளர்ச்சி எவ்வளவு வேகமானதாக இருக்கும் (Acute or Chronic), மேலும் எந்த வகை cell தொடர்பு கொண்டது என்று பல தொடர் விவரங்கள் எல்லாம் சேர்த்து மட்டுமே ஓரளவிற்கு அதன் அபாயம் பற்றி ஒரு அபிப்பிராயம் ஏற்படுத்தி கொள்ள முடியும். உண்மை இப்படி இருக்கும் பொழுது பொது வழக்கில் Cancer என்ற வார்த்தை பொத்தாம் பொதுவாக உடலின் எந்த உறுப்பு சம்பந்த பட்டதாக இருந்தாலும் அதன் அர்த்த தாக்கம் ஒன்றாகவே இருக்கும் நிலையில் தான் உள்ளது.

Cancer என்பது பொதுவாக ஒரு life threatening illness என்றும், புற்று நோய்க்கு உண்டான பல சிகிச்சை முறைகள் உடலுக்கு மிகுந்த வலி ஏற்படுத்த கூடியதாகவும், serious long term பக்க விளைவுகள் ஏற்படுத்துவதாகவும் உள்ளது என்பதும் பொதுவாக நம்பப்படும் கருத்துகலாக உள்ளன. நோயின் தாக்கம் உயிரையே அழிக்கும், அதற்கு நிச்சய நிரந்தர தீர்வு என்பது இல்லை, நோய்க்கு உண்டான சிகிச்சையே நோயை விட உயிருக்கு அபாயம் விளைவிக்க கூடியதாக இருக்கும் நிலையில், இந்த நோயை அணுகும் முறை, நோய் பற்றிய கருத்துகள் ஒன்றுக்கும் மேற்பட்டதாக இருப்பதில் ஆச்சரியம் இல்லை. Mainstream treatment approach என்பது allopathy (Chemo, Radiation-theraphy, Surgery) என்கின்ற முறைகளாகவே இருக்கின்றது.

Alternative Medcine என்பது பலவகை பலதரப்பட்ட சிகிச்சை முறைகளையும் குறிக்கும் ஒரு சொல். இது Conventional என்று நம்பப்ப்டும் சிகிச்சை முறைகள் அல்லாதவைகளை குறிக்கும் ஒரு பொது சொல். ஆயுர்வேதம், உந்நாநி போன்ற பல சிகிச்சை முறைகளும் இதில் உள் அடங்கியாவை. பொதுவாக conventional முறைகளால் கை விடப்பட்ட நோயாளிகளுக்கு ஒரு புகலிடமாக இவை உள்ளன. மேலும் சில சந்தர்ப்பங்களில் conventional சிகிச்சைக்கு ஆகும் செலவு மிக அதிகமாக இருக்கும் பட்சத்தில் இத்தகய alternative முறைகளை நாடி செல்கின்றனர்.

Conventional treatment approaches மிகுந்த சர்ச்சைக்கு உள்ளானவைகளாக இருக்கிறது. Pharma giants, public health costs, insurance firms என்று பல stakeholders, driven by profit motives என்கின்ற பல குறுகளின் கட்டுப்பாட்டுக்கு உட்பட்டு உள்ளது என்பது ஏற்று கொள்ள பாட வேண்டிய ஒரு உண்மை. அதே நேரத்தில் புற்று நோய் ஆராய்ச்சியில் முன்னேற்றம் ஏற்படுவதும், புற்று நோய் குணமாவதுமே அவர்கள் வியாபாரததிற்கு பாதிப்பு ஏற்படுத்தும் என்பதால் அத்தகய ஒரு விஷயத்தை விரும்பாத வில்லன் என்கின்ற kind of portrayal is to me kind of taking it to the extreme..

Cancerrealism என்னும் தத்துவத்தை அடிப்படையாக கொண்டு எழுதப்பட்ட இந்த நூலின் ஆங்கில மூலம் "The other face of Cancer" என்னும் புத்தகம் அதற்கு முன்பு 1973ல் "The nature of cancer" புத்தகமும் அதன் தொடர்ச்சியில் நடந்த ஆராய்ச்சி கட்டுரைகளின் தொகுப்பு என்றும் குறிப்பிடுகின்றனர் . இந்த தத்துவம் ஒரு மாற்று முறையாக மட்டும் இன்றி ஒரு radically different கண்ணோட்டம் ஆக இருக்கிறது. அது புற்று நோய் என்பது அறிதல் புரிதல் எல்லாவற்றிற்கும் அப்பாற்பதத்(ஏதோ பரம்பொருள் போல!!). அதை அதனுடைய போக்கிலேயே விட்டு விடுதல் உத்தமம் என்று சொல்கிறது.

நான் தேடி கண்டுபிடித்தது ஆங்கில மூலம் 1994ல் வெளியாகியுள்ளது. Oncology and field of cancer research has had some dramatic developements in the last years. அதனால் இந்த மாதிரியான ஒரு மருத்துவ புத்தகத்திற்கு எந்த காலகட்டத்தை குறிக்கிறது என்று தெரிவிப்பது மிக அவசியம். இந்த புத்தகத்தில் அதை ஒரு பொருட்டாக கூட எடுத்துக்கொண்டிருப்பதாக தெரியவில்லை. ஏதோ பேருக்கு கடைசி chapter 2000-2006 ஒரு கண்ணோட்டம் என்று உள்ளது.

Anyway even in the other chapters அவர்கள் நம்பும் இந்த கொள்கைக்கு நம்மை convince செய்வதற்கு உபயோகிக்கும் ஒரே யுததி.."Negative Criticism of the alternative mainstream treatement approaches". While i do not have any major issues with that attitude of theirs that does not really address the biggest challenge people face when they have to make a choice on any of the alternative ways to deal with their illness.

Cancer patientஓ அவர்களின் குடும்பத்தினரோ mainstream சிகிச்சை முறை வேண்டாம். ஏனெனில் அதில் பல அபாயங்களும் சிக்கல்களும் உள்ளன ஏதாவது ஒரு மாற்று சிகிச்சை முறைகளை முயற்சி செய்து பார்ப்போம் என்று என்னும் பொழுது அவர்களுக்கு பெரும் தடையாக இருப்பவை

1. Lack of credible alternatives. I mean existence of an alternative which presents itself as a proper system. Most of the alternate approaches are extremely ad-hoc.

உதாரணத்திற்கு ஒரு நோயாளி ஒரு மாற்று சிகிச்சை முயற்சி செய்து பார்த்து விட்டு அதன் மீது திருப்தி இல்லை, அது கூட வேண்டாம் அந்த particular treatment center adminsitering such an approach மீது நம்பிக்கை இல்லை அதனால் வேறு ஒரு இடத்தில் அல்லது mainstream முறைக்கே திரும்ப வேண்டும் என்றால், currently the process is very cumbersome.

மேலும் மாற்று சிகிச்சை முறைகளோ அதை தேர்ந்தெடுததவர்கள் the process they had been through in convincing themselves to choose this approach, அதை பற்றிய சிந்தனைகள் பகிர்ந்து கொண்டால் மிக உபயோகமாக இருக்கும்.
Infact அந்த மாதிரி மாற்று முறைகள் முயற்சி செய்து அது பயனில்லை என்று உணர்ந்து அப்பொழுது அந்த guilt தவறு செய்து விட்டோமோ என்ற உணர்வை எப்படி ஆட்கொண்டார்கள் ஆகிய தகவல்களே மிக பயனுள்ளதாக இருக்கும்

நாம் நம்பி மண்டி இடும் யேசு சொன்னது நமக்கு அல்ல நம் எதிரிக்காக..

நக்ஷத்திரங்களோடு போட்டி இடும் வெளிச்சத்துண்டங்களை
வானில் இருந்து பொழியும் வெடி குண்டு மழையை ரசிப்போம்
சீறிப்பாயும் போர் விமானங்கள்.. ஆஹா.. என்னவேகம்.. என்ன வேகம்..

அந்த ஏவுகனைகள் அது காலத்தின் ஆச்சரியம்

Diagnosis of cancer at any stage of life can be extremely overwhelming and traumatic. This experience is even more dramatic in the case of adolescents and young adults (approximately 15 to 35 years old), due to the developmental life cycle stage that they belong to. This particular group is already dealing with a range of issues related to education, career, and relationship and all that shapes up ones personal identity. Hence the diagnosis of cancer, which means a beginning of a long process of painful treatment and years of uncertainty, makes the additional burden of having to deal with cancer, an extraordinary and challenging time for them.

Even medically, this specific group seems to be unique in the distribution of the types of cancer, prognosis and causal factors. Majority of the cancer appearing in this age group are spontaneous, unrelated to either carcinogen in the environment or family cancer syndromes. The more disturbing fact is that there has been a lack of progress in the survival improvement, relative to all other ages in the last quarter century globally. National Cancer Institute, USA has recently acknowledged that this vulnerable age group is one of the under-represented in cancer studies and seeks to redress it.

This write-up is about an interactive play which aims to harness the power of the video game technology to engage young people with cancer, and deliver a positive health outcome. The name of the game is Re-Mission and the organization which developed the game is HopeLab.

Hopelab is a not for profit organization, which uses research based techniques and comes up with innovative ways to improve the quality of life of young adults with chronic illnesses. Re-mission is their first product aimed at cancer affected.

The game takes the player on a journey through the body of a fictional cancer patient, who represent young cancer patients with different types of cancer. Players control nanobot named Roxxi who destroys cancer cells, battles bacterial infections and manages realistic life threatening side effects associated with cancer treatment.

The game was designed to be fun and at the same time, impart knowledge about cancer, treatment regimens and through this knowledge aims to improve one’s confidence in their ability to manager their illness. The feeling of power over their illness that the children gain, as they blast away cancer cells in the game is in essence what the game aims to achieve.

The game was developed by a team of individuals with expertise in a wide range of disciplines, including research, psychology, game development and nutrition. The development team has worked closely with health cancer professionals to ensure that the medical terminologies used in the game were accurate, and that the goals of each missions represented a medically valid problem. They had also adopted an interactive game development process, to incorporate the input of young people with cancer, thereby ensuring that the game meaningfully reflect their perspectives. While there is an emphasis that the end product needs to be a fun game, it should not trivialize the larger objective that it is aimed at.

The developers of this game are also committed to the study of their interventions through outcome studies to ensure that the game developed is effective on its target audience. The study focused on the effect of the game and its influence on adolescents and young adults on the following aspects

- Adherence to cancer medications
- Knowledge about cancer information embedded in the game
- Quality of life
- Individuals belief in his/her ability to take a certain course of action in a challenging situation(the disease of cancer in this instance)

They report that the result of the outcome studies has been quite encouraging and these study and finding are openly shared with. I have had some personal interaction with the game developers and find them to be quite open to incorporating changes, adding new aspects to the game and demonstrate self-critical attitude on deciding about its effectiveness.

The PC-version of the game is made available to young people with cancer, free of charge through their website (remission.net). This site also provides an interactive, online community where teens and young adults with cancer can share information and support one another.

In addition they intend applying the insights gained from the development and study of this game to come up with similar technology-based approaches that affects young people with chronic illness. They are working on the following illnesses currently

- Obesity (Ruckus the game is available already)
- Sickle cell disease
- Major depressive disorder
- Autism

I intend to contribute for their outcome studies in the following way. I plan to organize a demonstration show on the game to a group of young cancer patients. Based on the reaction and relevance that they feel this in our socio-cultural context, we can organize an outcome study through a set of questionnaires to understand their reaction to the game. This input can be provided to the game developers and if they feel it makes sense they could incorporate it into the game.

Video game development is an extremely cost intensive activity. This would be not possible without huge financial backing and support. But certain aspects of this development process, like the knowledge about cancer information availability through an interactive medium, and the usefulness of that knowledge in enabling people to better deal with their illness can be made use and tried on
alternate techniques (online audio-visual tools as approaches to impart cancer information). Individual PC access for people in most hospitals can be an issue, but making available a common PC loaded with the game should be feasible.

With changing lifestyle and food habits, obesity among the kids and adults is also a major health concern in India. Hence their forthcoming product targeted at obesity can have a viable market in India. Hence an engagement with the game developers on their development methodologies can also make business sense.

Novartis: Patent Issue for Glivec

A Chennai court has rejected a challenge to the patent law filed by Novartis in a law-suit against the Indian government, the Cancer Patient Aid Association (CPAA) and some generic drug manufacturers in India. While the aid organizations have declared the ruling as a victory for the “right of patients over patents”, Novartis claims that the verdict will have long term negative consequences for the development of better medicines.

Novartis had claimed a patent for their anti-cancer drug Gleevec (known in India as Glivec). The cost of Glivec for a patient for one year is around 1.4 million INR, while other companies sell the generic version of the drug at about 1/10th of that price. The patent was denied by the Court on the basis of Section 3d of the Indian patent law which disallows patents for incrementally small innovations.
Had the case gone in favor of Novartis, it would have prevented the generic manufacturers from producing the drug at all. This case was very keenly followed by the international pharmaceutical industry, global relief organizations working for greater access to public health and generic drug manufacturers. There are about 9000 patent applications pending before the courts, most of them reflecting a scenario similar to this one. The way these cases are settled would thus have a profound influence on the production and sale of drugs for a wide range of diseases affecting the world.

This issue involves many powerful players with conflicting interests, with each trying to influence patent protection clauses for their own benefit. This article is an attempt to understand the view points of these various stake holders on the broader issues of patent protection and its consequences to (a) research and development investments by pharma firms, and (b) access to life saving drugs by the needy.

Linked to this is the vibrant growth of Indian generic drug manufacturers’ role in producing medicines for export to developing countries, and even to developed nations. As all these issues are inter-connected I will address them as a whole and, when it is relevant , discuss specific items. This article is organized as follows: I first present brief background information to understand the issues and then present different view points, and finally offer my perspective on the matter.

To start with, let us understand the issue of pharmaceutical patents in the Indian context. A patent is an exclusive right granted by the Government for an invention. This invention can either be (a) a product or (b) a process that offers a new way of doing or making something. Once the patent is granted, the invention cannot be commercially made, used, distributed or sold without the patent owner’s consent. The validity of the patent is for a fixed period, usually 20 years, during which the patent holder can exclusively profit from the invention. Therefore, it is understandable that large amounts of money are spent to obtain and protect patents.

Patents for pharmaceutical substances can be of two types (a) Product patents, which are given for a given chemical substance, say a drug, or (b) Process patents, which are for a specific way of manufacturing the substance. In the former case, the product patent allows the patent holder to profit exclusively from the drug, whose manufacture by any method is disallowed to others. In the process patent, however, others are forbidden only from producing the drug by the patented method but are free to manufacture it by another process of their own devising, which in turn could be patented.

For a long period of time, India granted only process patents for pharmaceuticals. Therefore pharma companies were free to devise a non-infringing process to manufacture a drug even if it was protected by a (process) patent in India. This changed when India became a member of WTO. As part of its obligation to comply with the TRIPS agreement, the Indian Patent Act now allows a product patent to be granted for a period of 20 years if it satisfies internationally accepted criteria for patentability. However, the amended law also includes, in Section 3(d), a provision that is unique to the Indian Act which states that patents would not be given for “incremental innovations”, i.e., new forms, uses or minor modifications of existing drugs unless they have a demonstrated greater “efficacy” than the earlier known one.

Given the current evolving phase of Indian patent laws, and the ambiguity of terms like “incremental” and “efficacy”, Novartis and other multinational drug manufacturers, as well as the groups opposing their patent claims, strengthen their arguments through self-serving interpretations of the Act. These technicalities and the nuances of patent laws appear obscure and are not the primary interest of this article. Those interested can refer to Novartis for Novartis’ point of view and to Medicine Sans Frontiers for the alternate argument.

The main concern of this article is to reconcile two opposing views. On one hand many people believe that is ethically untenable that patent law should offer the inventor company exclusive rights and monopoly on their invented product, giving them the freedom to make large amounts of profits from their product at a price fixed by their own choice, thereby leaving people who cannot afford the medicine to suffer, or even to die. On the other hand, pharmaceutical industries claim that protecting innovation, and the gain through innovative products, is the foundation for R&D investments made in the pharmaceutical sector. Without patent protection, the latter view goes, there would be no innovation and no new medicines.

There do exist some provisions which individual governments can exercise to provide medicines for those who cannot afford the prices dictated by the pharma companies. These are in place, at least theoretically, to enable the supply of essential medicines to poor countries and poor people. Section 3d of the Indian patent law is one such provision introduced by the Indian government for public health safeguards. This clause forbids the issue of patents for incremental innovation.

There is no clear consensus on what actually falls into the purview of incremental innovation. Section 3d is aimed to protect genuine improvement by barring frivolous tweaking being passed under the garb of innovation. It was introduced to prevent a practice termed "ever greening"--- i.e., drug companies making minor modifications to existing drugs so as to re-patent them at the end of their patent period, and selling them as new and improved products.

However, Novartis has argued that most medical progress happens through incremental innovation. And if the Indian law does not recognize these advances, it would negatively affect the development of better medicines.

On the other hand, Medicine Sans Frontiers, one of the leading NGO voices against Novartis, feels that allowing patents on minor modifications would actually be a disincentive for companies to address new medical challenges, for it would be more profitable to work on minor changes for patents than in investing on breakthrough innovation. And as an aside, it also argues that pharmaceutical research is driven by market potential, and as people in developing countries do not have the purchasing power to attract research funds for the serious diseases that affect them, pharmaceutical research primarily focuses on anti-wrinkle creams and anti-obesity drugs.

However, while I admit that research funding decisions are based on a cost-benefit analysis to maximize profit earnings, I believe that the typical culture of a research organization will certainly not be driven by such narrow attitudes. Furthermore, working towards incremental innovation and original inventions are usually not two exclusively different paradigms. In fact, most breakthrough inventions are achieved as one quantum leap in an incrementally innovational process.

As regards the other problem, of research focus areas not being relevant to the concerns of developing nations, this could be better achieved by working out innovative collaborations with pharma companies, than by denying them patent protection and depriving them of their profits.

Let us consider the Indian example. For a pharmaceutical company, the Indian market is not a homogenous entity. While there exists a large population of poor people with limited or no access to health care, there also exists a sizeable affluent population. A blanket protection against high drug prices, in the name of the poor, resulting in medicines being offered at cheap prices to affluent people cannot be a rational policy. Hence the Indian government can try to work on a differential pricing arrangement with pharma companies for the different markets that India comprises. The Indian government (or alternate monitoring/regulatory global bodies) can in return persuade the pharma companies to invest the profits gained from the affluent section of its population in medical research of concern to the country.

Research organizations like Novartis should work collaboratively with generic drug manufacturers, without claiming monopoly rights for critical drugs, to enable greater medical access to the entire population. The generic pharma companies in turn should reward the inventor by sharing a part of their profits for drugs within the patent regime sold in developing countries and the patent-expired drugs they sell to developed nations. Both players can thereby evolve a workable business strategy based on mutual trust.

Let us now consider the much-discussed issue of public access to drugs. Sustainable access to medicines in developing countries is a complex issue. Improving access to healthcare depends on a variety of factors
existence of trained healthcare staff and infrastructure
accessibility of the healthcare facility and quality of care
availability of affordable medicines
cultural acceptability of treatment(e.g. Islamic groups opposition to the polio vaccination drive in India recently)
and other factors.

The availability of medicines at reduced, off-patent, prices is just one aspect to this complex problem. That said, it is however a significant factor and a genuine commitment to address it is needed.

In the Glivec case, Novartis asserts that it is strongly committed to ensure that all patients have access to the medicine they need, and claims that 99% of the people who need Glivec are given the medicine free of cost through the Glivec International patient assistance program(GIPAP). However, this cannot be extended across the spectrum of other patented medicines. I believe corporate donations are not a sustainable solution to the problem. On this issue, all international pharmaceutical companies should come forward to work with government or non-governmental organizations for greater public access of patented medicines. This should not be just regarded as a public relations exercise to demonstrate their corporate social responsibility. A few things that these pharmaceutical companies can do are to

· offer not to file for patent protection for certain drugs in very poor countries and work with the respective government to enable access to the medicines that are needed locally. Such an act will gain them greater respect and credibility in the eyes of public.

· collaboratively work with drug manufacturers in poor nations to deal with
Infrastructure and stock-out contingencies.

An open attitude harmonizing corporate patent profit objectives with the privileges conferred to generic drug manufacturers, and concern for a general access to health care with an acknowledgement of the obvious buying power of the Indian affluent class, would facilitate policy development on a case by case basis for individual drugs. Upon such learning from individual cases can be evolved a generic strategy to attack this most important health case issue of our time.
